新Bobath技术联合上肢智能等速训练器对脑卒中后上肢痉挛性偏瘫患者康复效果的影响

目的 探讨新Bobath技术联合上肢智能等速训练器对脑卒中后上肢痉挛性偏瘫患者康复效果的影响.方法 前瞻性纳入于2021年5月至2023年5月至河南省平煤神马医疗集团总医院就诊的96例脑卒中后上肢痉挛性偏瘫患者进行研究,依据随机数字表法将其分为对照组和观察组,各48例.对照组接受新Bobath技术干预,观察组在对照组基础上联合上肢智能等速训练器干预,两组均连续干预4周.对比干预前、干预4周后上肢功能[Fugl-Meyer运动功能评估表(FMA)、改良Ashworth痉挛评价量表(MAS)]、神经功能[美国国立卫生研究院卒中量表(NIHSS)]、日常生活能力[改良巴氏指数评定表(MBI)].结果 干预4周后,两组FMA评分上升,且观察组高于对照组(P<0.05).干预4周后,两组MAS评分下降,且观察组低于对照组(P<0.05).干预4周后,两组NIHSS评分均下降,且观察组低于对照组(P<0.05).干预4周后,两组MBI评分均上升,且观察组高于对照组(P<0.05).结论 新Bobath技术联合上肢智能等速训练器可显著改善脑卒中后上肢痉挛性偏瘫患者上肢功能和神经功能,降低上肢偏瘫对日常生活的影响.
Impact of the New Bobath Technology Combined with an Upper Limb Intelligent Isokinetic Training Device on the Rehabilitation Effect of Upper Limb Spastic Hemiplegia Patients After Stroke
Objective To explore the impact of the new Bobath technology combined with an upper limb intelligent isokinetic training device on the rehabilitation effect of upper limb spastic hemiplegia patients after stroke.Methods A prospective study was conducted on 96 patients with upper limb spastic hemiplegia after stroke who were admitted to Henan Pingmei Shenma Medical Group General Hospital from May 2021 to May 2023.They were divided into a control group and an observation group using a random number table method,with 48 patients in each group.The control group received intervention with the new Bobath technology.The observation group was intervened with an upper limb intelligent isokinetic training device on the basis of the control group,and both groups were continuously intervened for 4 weeks.The upper limb function[Fugl-Meyer assessment scale(FMA),modified Ashworth spasticity scale(MAS)],neurological function[National Institutes of Health stroke scale(NIHSS)]and activity of daily life[modified Barthel index(MBI)]were compared before and 4 weeks after intervention.Results After 4 weeks of intervention,the FMA scores of both groups increased,and the observation group was higher than the control group(P<0.05).After 4 weeks of intervention,the MAS scores of the two groups decreased,and the observation group was lower than the control group(P<0.05).After 4 weeks of intervention,the NIHSS scores of both groups decreased,and the observation group was lower than the control group(P<0.05).After 4 weeks of intervention,the MBI scores of both groups increased,and the observation group was higher than the control group(P<0.05).Conclusion The combination of new Bobath technology and upper limb intelligent isokinetic training device can significantly improve upper limb function and neurological function in patients with spastic hemiplegia after stroke,and reduce the impact of upper limb hemiplegia on daily life.
