Correlation Between Changes in Postoperative Thromboelastogram Parameters and the Occurrence of Osteoarthritis in Patients with Tibial Plateau Fractures
Objective To observe the changes in thromboelastogram parameters in patients with tibial plateau fractures after surgery,and analyze their correlation for the occurrence of postoperative osteoarthritis in patients with tibial plateau fractures.Methods A prospective cohort study was conducted to select 150 patients with tibial plateau fractures admitted to Puyang People's Hospital from December 2020 to December 2022.All patients received small incision surgery guided by the knee arthroscopy.The parameters of thromboelastogram[blood coagulation time(K value),coagulation reaction time(R value),coagulation angle(Angle),maximum amplitude of thrombus(MA value)]were detected immediately after surgery.The incidence of osteoarthritis was observed at the return visit 3 months after surgery,and the correlation of thromboelastogram parameters and the occurrence of osteoarthritis was analyzed.Results Three months after surgery,28 out of 150 patients with tibial plateau fractures developed osteoarthritis(18.67%),while 122 out of them did not develop osteoarthritis(81.33%).The K and R values of the thromboelastogram parameters in the occurrence group were lower than those in the non occurrence group,and the Angle and MA values were higher than those in the non occurrence group(P<0.05).The results of point two column correlation analysis showed that the K and R values of thromboelastogram parameters were negatively correlated with postoperative osteoarthritis in patients with tibial plateau fractures(r<0,P<0.05),while the Angle and MA values of thromboelastogram parameters were positively correlated with postoperative osteoarthritis in patients with tibial plateau fractures(r>0,P<0.05).The receiver operating characteristic(ROC)curve was drawn,and the results showed that the thromboelastogram parameter predicted the area under curve(AUC)of postoperative osteoarthritis in patients with tibial plateau fractures,which was greater than 0.7 and had certain evaluation value.Conclusion The parameters of thromboelastogram are closely related to the occurrence of postoperative osteoarthritis in patients with tibial plateau fractures,and observing the changes in the K value,R value,Angle,and MA value of thromboelastogram parameters has important value in predicting the occurrence of postoperative osteoarthritis in patients with tibial plateau fractures.
