Effect of Combination of Esomeprazole and Terlipressin on Hepatic Venous Pressure Gradient in Cirrhotic Patients with Upper Gastrointestinal Bleeding
Objective To analyze the effect of esomeprazole combined with terpressin on hepatic venous pressure gradient after treatment of upper gastrointestinal bleeding in cirrhosis.Methods From May 2020 to October 2022,a total of 93 patients with cirrhosis of the upper digestive tract hemorrhage were selected and divided into two groups according to lottery.The observation group(47 patients)were treated with terpressin combined with esomeprazole,the control group(46 patients)were treated with somatostatin combined with esomeprazole.Efficacy,clinical indicators,inflammatory factors,hepatic venous pressure gradient and adverse reactions in two groups were compared.Results Free hepatic venous pressure(FHVP),hypersensitive C-reactive protein(hs-CRP),tumor necrosis faction-α(TNF-α),plasma cortisol(Cor),urine volume,serum creatinine and mean arterial pressure in the observation group were higher than those in the control group,and wedging hepatic venous pressure(WHVP)level was lower than those in the control group(P<0.05).There was no statistical significance in hemostatic effect and recurrence rate of hemorrhage between the two groups(P>0.05).Conclusion Esomeprazole combined with terpressin has a good effect on upper gastrointestinal bleeding in cirrhotic patients,can improve clinical indicators and inflammatory factors,and reduce the gradient of hepatic venous pressure.
