Relationship Between Preoperative Nasal Resistance and Postoperative Recurrence in Patients with Chronic Sinusitis and Nasal Polyps Undergoing Endoscopic Surgery
Objective To investigate the relationship between preoperative nasal resistance and endoscopic postoperative recurrence in patients with chronic sinusitis and nasal polyps.Methods A prospective study was conducted to select 127 patients with chronic sinusitis with nasal polyps who received endoscopic surgery in Department of Otorhinolaryngology,the Affiliated Hospital of Xinyang Vocational and Technical College from January 2020 to December 2021 as the study objects.Nasal resistance of patients was detected before surgery,treated with routine endoscopic surgery,and followed up for 1 year after surgery.Postoperative recurrence of patients was counted,the relationship between preoperative nasal resistance and postoperative recurrence of patients was analyzed,and receiver operating characteristic(ROC)curve was drawn to analyze the predictive value of preoperative nasal resistance in patients with chronic sinusitis with nasal polyps after endoscopic surgery.Results The average preoperative nasal resistance of 127 patients with chronic sinusitis with nasal polyp was(1.88±0.46)Pa·cm-3·s-1,and 45 cases(35.43%)recurred after operation.There were statistically significant differences in postoperative infection and smoking between the recurrence group and the non-recurrence group(P<0.05).Compared with non-recurrent group,the preoperative nasal resistance in the recurrent group was higher,with statistical significant differences(P<0.05).Multivariate logistic regression analysis showed that postoperative infection,smoking and preoperative nasal resistance were the risk factors for the recurrence of chronic sinusitis with nasal polyps after endoscopic surgery(OR>1,P<0.05).By point two-column correlation analysis,preoperative nasal resistance was positively correlated with endoscopic recurrence in patients with chronic sinusitis and nasal polyps(r>0,P<0.05).ROC curve results showed that preoperative nasal resistance predicted the area under the curve>0.7 in patients with chronic sinusitis with nasal polyps after endoscopic surgery,which had certain predictive value.Conclusion The greater the nasal resistance before endoscopic surgery,the higher the risk of postoperative recurrence in patients with chronic sinusitis and nasal polyp.The level of preoperative nasal resistance can be used as a predictor of postoperative recurrence in patients with chronic sinusitis and nasal polyp.
