Effect of the Ratio of Cholinesterase to Albumin on the Prognosis of COPD Patients with Type Ⅱ Respiratory Failure After Ventilation Treatment
Objective To analyze the effect of serum cholinesterase(CHE)to albumin(ALB)ratio on the prognosis of patients with chronic obstructive pulmonary disease(COPD)complicated with type Ⅱ respiratory failure treated with ventilation.Methods A prospective research method was used to select 110 patients with COPD complicated with type Ⅱ respiratory failure who were treated with bilevel positive airway pressure ventilation ventilator(BiPAP)in Central Hospital,Jiaozuo Coal Industry(Group)Co.,LTD.from May 2020 to April 2023 as the study subjects.Before treatment,the levels of CHE and ALB in patients were measured and the ratio was calculated.Based on their prognosis,they were divided into a good prognosis group and a poor prognosis group.CHE,ALB,CHE/ALB and baseline data were compared in both groups.Logistic regression test was used to analyze the factors affecting the outcome of ventilation therapy,and the receiver operating characteristic(ROC)curve was drawn to analyze the predictive value of CHE,ALB and CHE/ALB on the prognosis of ventilation therapy.Results Among the 110 patients,30 died(27.27%).There was a statistical significant difference in serum CHE,ALB,and CHE/ALB in both groups(P<0.05).According to logistic regression analysis,serum CHE,ALB,and CHE/ALB were influencing factors on the prognosis of patients undergoing ventilation therapy(P<0.05).ROC showed that serum ALB had a low predictive value for the prognosis of patients undergoing ventilation therapy,with an area under the curve(AUC)of 0.624.Serum CHE and CHE/ALB had moderate predictive value for the prognosis of patients undergoing ventilation therapy,with AUC of 0.784 and 0.804,respectively,with CHE/ALB having the highest AUC.Conclusion CHE/ALB can serve as a predictive indicator for the prognosis of ventilation therapy for COPD with type Ⅱ respiratory failure.
