Predictive Value of Combined Determination of Serum 25 Hydroxy Vitamin D and Glycated Hemoglobin A1c Levels for Foot Infection in Diabetic Patients
Objective To investigate the predictive value of combined detection of serum 25 hydroxy vitamin D[25(OH)D]and glycated hemoglobin A1e(HbA1c)levels in the occurrence of foot infection in diabetic patients.Methods From January 2021 to September 2023,86 diabetic patients without foot infection were selected as the diabetes group and 86 diabetic patients with foot infection were selected as the diabetic foot group in the First Affiliated Hospital of Xinxiang Medical College.The levels of serum 25(OH)D and HbA1c were compared between diabetes group and diabetic foot group.The status of serum 25(OH)D and control of HbA1c were compared between diabetes group and diabetic foot group.The relationship of different 25(OH)D status,different HbA1 c control with diabetic foot infection were analyzed.The interaction of serum 25(OH)D and HbA1 c levels on the incidence of diabetic foot infection was analyzed.The predictive value of combined detection of serum 25(OH)D and HbA1c for foot infection in diabetic patients was analyzed.Results The 25(OH)D in diabetic foot group was lower than that in diabetic foot group,HbA1c was higher than that in diabetic foot group(P<0.05).The deficiency rate and severe deficiency rate of serum 25(OH)D,poor control rate and worse control rate of HbA1c in diabetic foot group were higher than those in the diabetes group(P<0.05).The risk of foot infection in patients with 25(OH)D deficiency and severe deficiency were 3.373 and 5.058 times,which were higher than that in patients with 25(OH)D deficiency(P<0.05).The risk of foot infection in patients with poorly controlled and poorly controlled HbA1c were 3.002 and 5.220 times,which were higher than that in patients with ideal HbA1c control(P<0.05).The interaction OR value of serum 25(OH)D status and HbA1c control on the occurrence of diabetic foot infection was less than the product of OR value of 25(OH)D alone and the OR value of HbA1c,which satisfied the submultiplication model.The interaction coefficient γ>1,the status of serum 25(OH)D and control of HbA1c had an interaction effect on the incidence of diabetic foot infection(P<0.05).The AUC of 25(OH)D and HbA1c combined detection for predicting foot infection in diabetic patients was 0.909,which was higher than that of 25(OH)D and HbA1c alone 0.764 and 0.779(P<0.05).Conclusion Diabetic foot infection patients have lower serum 25(OH)D level and higher serum HbA1c level.Serum 25(OH)D deficiency and severe deficiency and poor control of HbA1c are related to the occurrence of diabetic foot infection.The combined detection of serum 25(OH)D and HbA1c levels has a high predictive value for the occurrence of foot infection in diabetic patients.
