Application Effect of Continuous Care Model Under Multidisciplinary Collaboration in Patients with Chronic Obstructive Pulmonary Disease
Objective To explore the application effect of continuous care model under multidisciplinary collaboration in patients with chronic obstructive pulmonary disease(COPD).Methods A total of 86 COPD patients discharged from the Second Out-Patient of Henan Province-Direct Organizations from March 2020 to September 2022 were selected as the study objects,and were divided into observation group(43 cases)and control group(43 cases)by random number method.The control group was given routine extended care,and the observation group was given continuous care mode under multidisciplinary collaboration on this basis.6-minute walking distance(6MWD),forced vital capacity(FVC)and forced expiratory volume in first second(FEV1)were used to evaluate cardiopulmonary function,COPD assessment test(CAT)was used to evaluate symptom control,and generic quality of life inventory(GQOLI-74)was used to evaluate quality of life.Results After extended nursing intervention,cardiopulmonary function indexes of 6MWD,FVC and FEV1 in both groups were increased,and the levels of observation group was higher than those of control group(P<0.05).The symptom control of observation group was better than that of control group(P<0.05).The scores of physical function,psychological function,social function and material life status in both groups were increased,and the GQOLI-74 scores of observation group was higher than those of control group(P<0.05).Conclusion The continuous care model under multidisciplinary collaboration could effectively improve the cardiopulmonary function of COPD patients,enhance the quality of life after discharge,and have a remarkable effect on symptom control.
