Relationship Between Levels of Peripheral Blood NLRP3,Caspase-1,ASC,IL-1βand Heart Failure in Patients with Acute Myocardial Infarction After PCI
Objective To analyze the relationship between NOD-like receptor protein 3(NLRP3),apoptosis-associated speck-like protein containing a CARD(ASC)and cysteine proteinase-1(Caspase-1)in peripheral blood mononuclear cells(PBMC),peripheral blood interleukin-1 β(IL-1 β)and heart failure(HF)in patients with acute myocardial infarction(AMI)after percutaneous coronary intervention(PCI).Methods A total of 105 patients with AMI undergoing PCI in the hospital were retrospectively enrolled between March 2021 and December 2022.According to presence or absence of HF within 6 months after surgery,they were divided into HF group(35 cases)and non-HF group(70 cases).The expression levels of NLRP3,ASC and Caspase-1 in PBMC were detected by Western blot,levels of cardiac function indexes[serum cardiac troponin Ⅰ(cTn Ⅰ),plasma brain natriuretic peptide(BNP)]and inflammation index 1β(IL-1β)were detected by enzyme-linked immunosorbent assay,and left ventricular ejection fraction(LVEF)was detected by color Doppler ultrasonic diagnosis apparatus.The correlation between NLRP3,ASC,Caspase-1 in PBMC,cardiac function indexes and the inflammation index was analyzed by Pearson correlation analysis.The diagnostic value of NLRP3,ASC,Caspase-1 and IL-1 β for HF after PCI was evaluated by receiver operating characteristic(ROC)curve.Results The expression levels of NLRP3,ASC and Caspase-1 in PBMC,and levels of serum cTnⅠ,plasma BNP and serum IL-1β in HF group were higher than those in non-HF group,while LVEF was lower than that in non-HF group(P<0.05).The results of Pearson correlation analysis showed that the expressions of NLRP3,ASC and Caspase-1 proteins were positively correlated with cTnⅠ,BNP and IL-1β levels(P<0.05),while negatively correlated with LVEF(P<0.05).IL-1β level was positively correlated with cTnⅠ and BNP levels(P<0.05),while negatively correlated with LVEF(P<0.05).ROC curves analysis showed that AUC values of NLRP3,ASC,Caspase-1,IL-1 β and combined detection for predicting HF were 0.711,0.729,0.748,0.724 and 0.836,respectively(P<0.05).AUC of combined detection was greater than that of single index(P<0.05).Conclusion The preoperative NLRP3,ASC,Caspase-1 and IL-1 β have high predictive value for HF in AM patients after PCI.The higher the expression levels of NLRP3,ASC and Caspase-1 and the severer the inflammatory response,the higher the risk of postoperative HF.
