Correlation Factors of Attached Gingival Width After Free Gingival Flap Transplantation Based on LASSO-Logistic Model
Objective To investigate the factors related to attached gingival width after free gingival flap transplantation. Methods A total of 220 patients undergoing free gingival flap transplantation in the First Affiliated Hospital of Zhengzhou University from January 2018 to December 2022 were selected as the study objects, and were divided into>2 mm group and 2 mm group according to the attached gingival width 6 months after surgery.LASSO-logistic regression model was constructed to screen independent variables. The optimal harmonic coefficient λ was selected by the ten-fold cross-validation method.Akaike information criterion(AIC)and Bayesian information criterion(BIC)were used to evaluate the goodness of fit of LASSO-logistic model, full variable Logistic model and step logistic model. Results Six months after operation,10 cases of 220 patients who underwent free gingival flap transplantation were lost to follow-up.The attached gingival width was(3.65±1.10)mm,and the proportion of attached gingival width ≤2 mm was 36.67%.There were statistically significant differences in probing depth(PD),plaque index(PLI),gingival recession(GR),graft thickness, smoking history, bleeding on probing(BOP),bad brushing habits, lip and tongue perforation wearing accessories, periodontal biotype, operation timing and postoperative infection between the two groups(P<0.05).LASSO-logistic equation showed that timing of operation(OR:4.413),graft thickness(OR:0.556),periodontal biotype(OR:3.824),PLI(OR:4.911),BOP(OR:4.528),smoking history(OR:5.368),postoperative infection(OR:5.513)was the influential factor of attached gingival width after free gingival flap transplantation(P<0.05),and BIC and AIC were the smallest. Conclusion The LASSO-logistic model based on the factors related to the attached gingival width after free gingival flap transplantation has a good fitting accuracy, which can provide a favorable reference for clinicians to master the method of attached gingival widening and maintain the long-term stability of implant denture.
