Value of Ultrasound Diagnosis in the Evaluation of Axillary Lymph Node Metastasis of Breast Cancer and Imaging Manifestations
Objective To analyze the value of ultrasonography in evaluating whether axillary lymph node metastasis occurs in breast cancer and its imaging manifestations. Methods A total of 100 patients with breast cancer admitted to Anyang Cancer Hospital from January 2022 to June 2023 were selected for observation. The axillary lymph nodes of patients were examined by ultrasound, and the imaging manifestations were sorted out.The pathological diagnosis results of patients were taken as the"gold standard"to calculate the coincidence rate of ultrasound diagnosis on the axillary lymph node conversion of patients.Combined with the imaging results, the cortical morphology and blood flow signals of patients were sorted out. Results After axillary lymph node metastasis in breast cancer patients under ultrasound imaging, sentinel lymph nodes were abnormal, but the size and structure did not change significantly.Cortical localized protrusions, eccentric thickening or diffuse thickening(>3 mm).As the disease progresses, lymph nodes appeared circular or elliptical, with a length to diameter ratio of less than 2.After tumor cells destroy lymph nodes, the strong echogenicity of the lymph node hilum disappeared or shifted under imaging.Blood flow signals were chaotic and resistance increases.According to pathological diagnosis, among the 100 patients,47 had axillary lymph node metastasis(metastasis group)and 53 had no metastasis(non-metastasis group). The ultrasound diagnostic accuracy rate was 94.03%(43/47).The breast cortex morphology of patients in the metastasis group was mainly eccentric thickening type(61.70%),centripetal thickening type(23.40%),and narrow type(14.89%).The breast cortex morphology of patients in the non-metastatic group mainly showed centripetal thickening(26.42%)and narrowing(73.58%).There was a statistically significant difference in narrowing data between the groups(P<0.05).The blood flow distribution of patients in the metastasis group showed mixed type, peripheral type, scattered type, and lymph node type, with rates of 8.51%,53.19%,25.53%,and 12.77%,respectively.The blood flow distribution status of patients in the non metastatic group showed mixed type, peripheral type, scattered type, and lymph node type, which were 5.66%,5.66%,13.21%,and 75.47%,respectively.There was a significant difference in the data comparison of peripheral type and lymph node type between the groups(P<0.05).Conclusion The ultrasonic for the diagnosis of axillary lymph nodes in breast cancer patients has a high consistency with pathological diagnosis, and can be combined with the imaging results of ultrasonic diagnosis to make clear the cortical morphology and blood flow status of patients, which helps doctors to accurately understand the location of patients'lesions, and provides a reference for the development of subsequent treatment plans.
