Value analysis of EB virus antibody titer and EB-DNA load in diagnosis and treatment of IM patients
Objective To investigate the value of EB virus(EBV)antibody titer and EB-DNA load in the diagnosis and treatment of infectious mononucleosis(IM)in order to provide reference for the clinical diagnosis and treatment of IM.Methods Totally 145 IM patients admitted to our hospital from May 2020 to January 2023 were selected, and 145 non-IM patients were selected as normal control group during the same period. The positive rates of EBV antibody and EB-DNA load were compared between the two groups, and the diagnostic value of EBV antibody combined with EB-DNA load was analyzed. The influence factors of EB-DNA not turning negative after 7 days of treatment were analyzed. Results The positive rates of EBV-CA-IgM,EBV-CA-IgG and EB-DNA load in IM group were 86.90%,62.76%and 78.62%higher than those in normal control group 6.90%,15.86%and 7.89%,the difference was sta-tistically significant(P<0.05).The sensitivity and accuracy of EBV antibody and EB-DNA load in the diagnosis of IM were 97.24%and 91.72%,which were significantly higher than that of EBV-CA-igm,EBV-CA-igg,EBV-NA-igg and EB-DNA load alone(P<0.05).The initial Ct value and the proportion of interferon used in the non-negative group were lower than those in the negative group, and the neutrophil/lymphocyte and C-reactive protein were higher than those in the negative group, with statistical significance(P<0.05).Initial Ct value and use of interferon were the influencing factors for EB-DNA not to turning negative in IM patients after 7 days of treatment(P<0.05).Conclusion EBV antibody combined with EB-DNA load detection has a synergistic complementary effect in the diag-nosis of IM,which can significantly improve the sensitivity and accuracy. In addition, the initial Ct value and the use of interferon are related to the non-negative EB-DNA in IM patients.
