Case report and literature review of suppurative meningitis in children with leukemia caused by Bacillus cereus
Objective Investigate the clinical characteristics of suppurative meningitis in children with leukemia caused by Bacillus cereus and improve the understanding of this disease.Methods Clinical data of a child with leukemia and suppurative meningitis which caused by bacillus cereus admitted to Henan Provincial People's Hospital in August 2020 were retrospectively analyzed,and the relevant literatures were searched and reviewed.Results The boy,11 years old,was diagnosed with acute lymphoblastic leukemia for 11 months.This child had low-grade fever,nausea,and abdomi-nal pain after taking unclean food during the agranulocytosis phase after chemotherapy.Empiric anti-infective therapy was given,but the effect was poor.The child soon developed con-sciousness disturbance and dyspnea.Head CT examination indicated cerebral edema.Cerebrospinal fluid examination from lumbar puncture appeared to be yellow with white blood cells and proteins significantly increased.Blood culture suggested Bacillus cereus infection.Considered with the symptoms and laboratory examinations,the patient was suspec-ted to be suppurative meningitis caused by Bacillus cereus.Then the anti-infection treatment was adjusted to teicopla-nin,voriconazole,and meropenem,meanwhile endotracheal intubation ventilator was used to assist breathing.The child still had no obvious improvement,then discharged voluntarily due to financial problems.Conclusion Agranulocytosis in children with leukemia is a high-risk factor for infection with Bacillus cereus,which progresses rapidly and threatens the life of patients.Clinicians should be vigilant.
