Correlation between periodontal status and malnutrition inflammation atherosclerosis syndrome in hemodialysis patients
Objective To explore the relationship between oral periodontal status and malnutrition inflammation athero-sclerosis syndrome(MIA)in hemodialysis(HD)patients.Methods A total of 76 HD patients who met the inclusion and exclusion criteria at our hospital's hemodialysis center from December 2022 to December 2023 were selected as the research subjects.According to the diagnostic criteria for periodontal disease,they were divided into four groups:no periodontal disease,mild periodontal disease,moderate periodontal disease,and severe periodontal disease.Patients in each group were evaluated using the Geriatric Nutritional Risk Index(GNRI),serum leptin(LP),and Detection of nutritional relat-ed indicators such as human serum albumin(HSA)and hemoglobin(Hb),as well as serum hypersensitive C-reactive protein(hs-CRP),procalcitonin(PCT),and tumor necrosis factor-α(TNF-α)、Interleukin-6(IL-6)and other-related inflammatory indicators and atherosclerosis indicators such as pulse pressure index and vascular intima thickness were detected.A multi factor linear regression model was established to analyze the differences of nutritional status indicators,inflammatory indicators and atherosclerosis indicators in each group of HD patients,and to explore the potential promoting factors of MIA syndrome in HD patients.Results As the se-verity of periodontal disease increases,the levels of LP,HSA,Hb,and GNRI all significantly decrease,while hs-CRP,IL-6,TNF-α.The thickness of the carotid artery intima significantly increased,and the difference was statistically sig-nificant(P<0.01).The correlation analysis between periodontal disease and indicators such as GNRI,hs-CRP,and carotid intimal thickness shows that the severity of periodontal disease is related to hs-CRP,TNF-α There is a positive correlation with carotid intimal thickness,but a negative correlation with LP,HSA,Hb,and GNRI.Conclusion Im-paired periodontal health is very common in HD patients,and severe periodontal disease is an independent risk factor for the occurrence of MIA syndrome in HD patients.In clinical work,attention should be paid to the comprehensive interven-tion and treatment of periodontal disease and MIA syndrome in HD patients.
