Influencing factors and preventive measures of lower respiratory tract infection in laryngeal cancer patients after tracheotomy
Objective To investigate the influencing factors of lower respiratory tract infection in laryngeal cancer patients after tracheotomy,and to provide evidence for the formulation of preventive intervention measures.Methods A total of 158 patients with laryngeal cancer admitted to our hospital from January 2020 to December 2022 were selected.The infec-tion rate and pathogen distribution of lower respiratory tract were analyzed.The influencing factors of lower respiratory tract infection in laryngeal cancer patients after tracheotomy were analyzed by univariate and binary logistic regression analysis.Results Among 158 patients with laryngeal cancer,the incidence of lower respiratory tract infection was 13.29%(21/158).Twenty-eight pathogenic strains were isolated from 21 patients with lower respiratory tract infection,included 20 strain of Gram-negative(71.43%)and 8 strain of Gram-positive(28.57%).There were statistically significant differ-ences in clinical stage,operation method,operation time,ventilator use,gastric tube indignant,pulmonary disease,anti-biotic use time and hospital stay between lower respiratory tract infection group and uninfected group(P<0.05).The bi-nary logstics regression analysis showed that clinical staging(Ⅲ~Ⅳ stage),surgical method(total laryngectomy),the use of ventilator,operating time(≥4 h),antibiotic use time(>5 d)were the independent risk factors of lower respira-tory tract infection in laryngeal cancer patients after tracheotomy(P<0.05).Conclusion In the surgical treatment of la-ryngeal cancer patients,attention should be paid to the incidence of lower respiratory tract infection,screening of related risk factors,and reasonable preventive intervention measures should be taken to reduce the incidence of lower respiratory tract infection and improve the postoperative quality of life of laryngeal cancer patients.
