芪冬颐心颗粒治疗慢性心力衰竭患者的临床观察

摘要
目的 观察芪冬颐心颗粒对慢性心力衰竭(chronic heart failure,CHF)患者的临床疗效.方法 选取2022年1月至12月就诊于阜外华中心血管病医院CHF患者240例,随机分为2组(各120例),对照组给予标准的西医抗心衰治疗药物,治疗组在对照组基础上联合芪冬颐心颗粒,两组均治疗12周.对比两组治疗前后的临床疗效、心脏射血分数(LVEF)、B型尿钠肽(BNP)等心功能指标、运动耐量指标6 min步行试验(6MWD)、生活质量指标明尼苏达心力衰竭生活质量量表评分(MLHFQ)及主要心血管不良事件(MACE)发生情况.结果 治疗组的心功能分级和中医证候疗效改善总有效率分别为91.45％和87.17％,对照组为82.61％和78.26％,治疗组的心功能疗效改善总体有效率显著高于对照组(P＜0.05).与本组治疗前比较,两组患者治疗12周后,LVEF、6MWD指标均较治疗前增加,BNP水平、MLHFQ评分、LVEDD水平均较治疗前下降(P＜0.05).值得注意的是,治疗组LVEDD、BNP、MLHFQ的降低显著优于对照组(P＜0.05);且运动耐量6MWT和LVEF的改善程度显著优于对照组(P＜0.05).两组患者不良心血管事件发生没有明显差异(P＞0.05).结论 芪冬颐心颗粒联合西医标准治疗,能进一步改善CHF患者的心功能和中医证候评分,提高运动耐量和生活质量.
Abstract
Objective To observe the clinical effect of Qidong Yixin Granules in treatment of patients with chronic heart failure.Methods A total of 240 patients with CHF who were admitted to Fuwai Central China Hospital from January to December 2022 were divided into two groups(120 cases in each group).The control group received western medicine anti-heart failure drugs.The treatment group was combined with Qidong Yixin Granules on the basis of the control group.Patients in two groups were treated and followed up for 12 weeks.The clinical efficacy,cardiac ejection fraction(LVEF),B-type natriuretic peptide(BNP)and other cardiac function indexes,exercise tolerance index 6-minute walking test(6MWD),quality of life index Minnesota heart failure quality of life scale score(MLHFQ)and major adverse cardiovas-cular events(MACE)were compared between the two groups before and after treatment.Results The total effective rates of cardiac function classification and TCM syndrome improvement in the treatment group were 91.45％ and 87.17％,re-spectively,while those in the control group were 82.61％ and 78.26％.The general effective rate of improvement of heart function in the treatment group was significantly higher than those in controls(P＜0.05).After 12 weeks of follow-up,the LVEF and 6MWD of the two groups were higher than basal level,and the BNP level,MLHFQ score and LVEDD level were also lower than basal level(P＜0.05).It is worth noting that the decrease of LVEDD,BNP and MLHFQ in the treatment group was significantly higher than those in controls(P＜0.05).The improvement of exercise tolerance 6MWT and LVEF was significantly better than that of the control group(P＜0.05).There is no significant difference in MACE occur between the two groups(P＞0.05).Conclusion Qidong Yixin Granules combined with western medicine standard treat-ment can further improve the cardiac function and TCM syndrome score of CHF patients,and improve exercise tolerance and quality of life.
