目的 探讨手术范围和放疗对不同性别和年龄段T2N0M0 1期甲状腺乳头状癌(papillary thyroid carcino-ma,PTC)患者预后的影响.方法 应用回顾性研究方法,选取2008年7月-2012年3月濮阳油田总医院甲状腺血管外科收治及手术治疗的PTC患者417例,对比分析不同年龄段、不同性别PTC患者的10年生存率(OS),应用多变量COX回归分析T2N0M0 1期PTC患者预后的影响因素.结果 ①361例患者,10年全因生存310例,10 年 OS 85.87％;18～55 岁患者,10 年 OS 90.91％;＞55 岁患者,10 年 OS 72.16％.男性 10 年 OS 78.89％,女性10年OS 88.19％.②在＞55岁的年龄段,无放疗+甲状腺全切除术(total thyroidectomy,TT)和无放疗+TT,均是影响患者预后的危险影响因素(P＜0.05).③男性患者,无放疗+TT和无放疗+TT,均是影响患者预后的危险影响因素(P＜0.05).结论 对于T2N0M0 1期PTC患者,甲状腺全切除术联合放射治疗可提高年龄＞55岁患者(男性+女性组合队列)和男性患者(年龄≥18岁)的10年OS.
Effects of surgical scope and radiotherapy on prognosis of patients with T2NOM01 thyroid papillary carcinoma
Objective To investigate the effect of surgical scope and radiotherapy on the prognosis of T2N0M01 stage PTC patients of different sex and age.Methods The retrospective study method was applied.Totally 417 patients with surgical treatment of PTC were selected from Puyang oil field general hospital thyroid vascular surgery during July 2008 to March 2012.The 10-year OS with PTC patients of different ages and genders was compared and analyzed.Multivariate COX regression was used to analyze the prognostic factors of T2N0M01 PTC patients.Results ① Among 361 patients,310 were all cause survival in 10 years,and the OS was 85.87％in 10 years;Patients aged 18-55 years,10 years OS was 90.91％;＞55 year old patient with OS 72.16％for 10 years.Male 10 years OS was 78.89％;Female 10 years OS was 88.19％.② In the age group＞55 years old,no radiotherapy+TT and no radiotherapy+TT were risk factors affecting the prognosis of patients(P＜0.05).③ Male patients,no radiotherapy+TT and no radiotherapy+TT,were risk factors affecting the prognosis of patients(P＜0.05).Conclusion For patients with T2NOMO1 PTC,total thyroid-ectomy combined with radiotherapy can improve the 10-year OS of patients older than 55 years(male+female com-bined cohort)and male patients(age ≥ 18 years).
