Study on diagnostic value of ultrasonographic microvascular imaging for thyroid nodules in Elderly
Objective This study aimed to analyze the application value of Superb Micro-vascular Imaging(SMI)tech-nique in the differential diagnosis of benign and malignant thyroid nodules in the elderly. Methods A retrospective study was conducted on ultrasound data of 93 elderly patients with thyroid nodules who were diagnosed and treated at Henan Gen-eral Hospital from May 2021 to December 2022.By comparing and analyzing the ultrasound characteristics of conventional ultrasound(US)and SMI blood flow features, key factors influencing malignancy in elderly thyroid nodules were identified using multiple-factor logistic regression analysis. Results Among the 93 patients with a total of 108 nodules,41 patients had 43 malignant nodules, and 52 patients had 65 benign nodules confirmed by pathology. There were no significant differ-ences in age, gender, nodule location, and size between benign and malignant nodules(P>0.05).However,there were statistically significant differences in echogenicity, calcification, halo signs, and SMI blood flow characteristics(P<0.05).Multivariate logistic regression analysis revealed that SMI grade Ⅲ~Ⅳ blood flow distribution, aspect ratio≥ 1,and heterogeneous halo signs were independent influencing factors for predicting malignant thyroid nodules in the eld-erly(P<0.05).Conclusion SMI Ⅲ~Ⅳ blood flow characteristics can serve as characteristic ultrasound features of ma-lignant thyroid nodules in the elderly, providing additional evidence for clinical diagnosis and treatment, and playing a positive role in the differential diagnosis of benign and malignant thyroid nodules in the elderly.
