Clinical analysis of human herpesvirus encephalitis diagnosed by next-generation sequencing of cerebrospinal fluid
Objective To explore the diagnostic methods and the characteristics of cerebrospinal fluid examination in pa-tients with human herpesvirus encephalitis. Methods We diagnosed 10 cases of human herpesvirus encephalitis in our hospital by testing cerebrospinal fluid examples with next-generation sequencing technology and analyzed the clinical characteristics and laboratory test results(2018-2022).Results In ten patients with human herpesvirus encephalitis, there are 2 cases with human alphaherpesvirus-1,3 cases with human alphaherpesvirus-2 and 5 cases with varicella-zoster virus.Nine had headaches, five had fever, two had epileptic seizure, two had abnormal mental behavior and one got uroschesis.Six cases had elevated cerebral pressure.Moderate lymphocytic pleocytosis were detected in all cerebrospinal fluid examples.Increased lactate was detected in five examples. Conclusion Next-generation sequencing of cerebro-spinal fluid is a new and effective method for diagnosing human herpesvirus encephalitis. The evidently increased number of white blood cells in cerebrospinal fluid are not only in purulent meningitis and tuberculous meningitis, but also in some human herpesvirus encephalitis. Some patients may also be detected increased lactate in cerebrospinal fluid.
