注射用己酮可可碱联合依折麦布对脑梗死恢复期患者神经功能缺损及血清Hcy、LDL-C水平的影响

目的 分析注射用己酮可可碱联合依折麦布对脑梗死(cerebral infarction,CI)恢复期患者神经功能缺损及血清同型半胱氨酸(homocysteine,Hcy)、低密度脂蛋白(low-density lipoprotein,LDL)水平的影响。方法 2018年11月至2022年11月驻马店市中心医院CI恢复期患者314例,随机分为对照组、联合组,各157例。对照组采用依折麦布片治疗,联合组采用注射用己酮可可碱+依折麦布片治疗。比较两组临床疗效、美国国立卫生研究院卒中量表(NIHSS)、Barthel指数评定量表(BI)、简易智能状态量表(MMSE)、血液流变学指标[全血低切黏度(low blood viscosity,LBV)、血细胞比容(hematocrit,HCT)、血浆黏度(plasma viscosity,PV)、全血高切黏度(high blood viscosity,HBV)]、血清Hcy、LDL-C水平、神经因子[S-100蛋白(S100B)、脑源性神经生长因子(brain-derived neuotrophyic factor,BDNF)、神经元特异性烯醇化酶(neuron specific enolase,NSE)]水平。结果 联合组总有效率88。54%,高于对照组的79。62%(P<0。05);治疗前两组NIHSS、BI及MMSE评分比较无统计学意义(P>0。05),联合组治疗后BI、MMSE评分(79。32±6。96)分和(25。31±2。71)分,均高于对照组(P<0。05),联合组治疗后NIHSS评分(9。38±1。23)分,低于对照组的(12。28±1。37)分(P<0。05);治疗前两组HBV、PV、HCT及 LBV 比较无统计学意义(P>0。05),治疗后联合组为(4。13±0。56)mPa/s、(1。23±0。18)mPa/s、(41。37± 6。62)%和(7。65±2。53)mPa/s,均低于对照组(P<0。05);治疗前两组Hcy及LDL水平比较无统计学意义(P>0。05),联合组治疗后 Hcy 及 LDL 水平(19。35±3。54)mPa/s 和(2。95±0。62)mPa/s,均低于对照组(P<0。05);治疗前两组S100B、BDNF及NSE水平比较无统计学意义(P>0。05),联合组治疗后S100B、NSE水平(0。87±0。26)mg/L 和(7。35±1。82)ng/mL,均低于对照组(P<0。05),联合组治疗后 BDNF 水平(8。72± 0。91)ng/mL,高于对照组的(6。81±0。83)ng/ml(P<0。05)。结论 CI患者恢复期采用注射用己酮可可碱联合依折麦布治疗效果显著,可抑制血栓形成,促进血液循环、神经修复,还可改善认知功能,提高患者日常生活能力。
Effects of pentoxifroline injection combined with Ezetimibe on neurological impairment and serum levels of Hcy and LDL-C in convalescent cerebral infarction patients
Objective To analyze the effects of injection of pentoxifylline combined with ezetimibe on neurological defi-cits and serum homocysteine(Hcy)and low-density lipoprotein(LDL)levels in recovered patients with cerebral in-farction(CI).Methods From November 2018 to November 2022,314 patients with cerebral infarction(CI)in Zhuma-dian Central Hospital were randomly divided into control group and combination group,with 157 patients in each group.The control group was treated with ezetimibe tablets,while the combination group was treated with injection of pentoxifyl-line and ezetimibe tablets.Clinical efficacy between two groups,National Institutes of Health Stroke Scale(NIHSS),Barthel Index Rating Scale(BI),Mini State of Intelligence Scale(MMSE),hemorheological indicators[low blood vis-cosity(LBV),hematocrit(HCT),plasma viscosity(PV),high blood viscosity(HBV)],serum Hey,LDL-C levels Neuron specific enolase(NSE)levels,including S-100 protein(S100B),brain-derived neurotrophic factor(BD-NF),and neuron specific enolase were compared.Results The total effective rate of combined group was 88.54%,which was higher than that of control group(79.62%)(P<0.05).There was no significant difference in NIHSS,BI and MMSE scores between the two groups before treatment(P>0.05).After treatment,the BI and MMSE scores of the combined group were(79.32±6.96)and(25.31±2.71),which were higher than those of the control group(P<0.05).The NIHSS score of the combined group was(9.38±1.23),which was lower than that of the control group(12.28±1.37)(P<0.05).There was no significant difference in HBV,PV,HCT and LBV between the two groups before treatment(P>0.05),and the combined group after treatment was(4.13±0.56)mPa/s,(1.23±0.18)mPa/s,(41.37±6.62)%and(7.65±2.53)mPa/s.It was lower than that of control group(P<0.05).There was no significant difference in Hcy and LDL levels between the two groups before treatment(P>0.05).After treatment,the Hey and LDL levels in the combination group were lower than those in the control group(P<0.05)at(19.35±3.54)mPa/s and(2.95±0.62)mPa/s.There was no significant difference in S100B,BDNF and NSE levels between the two groups before treatment(P>0.05).After treatment,the S100B and NSE levels in the combination group were lower than those in the control group(P<0.05)(0.87±0.26 mg/L and 7.35±1.82 ng/mL).After treatment,the level of BDNF in combination group was(8.72±0.91)ng/mL,which was higher than that in control group(6.81±0.83)ng/mL(P<0.05).Conclusion In the convalescent period of CI patients,pentoxifroline injection combined with ezetimibe has a significant effect,which can inhibit thrombosis,promote blood circulation,nerve repair,improve cognitive function and enhance patients'ability of daily life.
