改良经腹膜外全筋膜内腹腔镜前列腺癌根治性切除对局限性前列腺癌的疗效

摘要
目的 探讨改良后经腹膜外全筋膜内腹腔镜前列腺癌根治性切除对局限性前列腺癌的应用价值.方法 将2021年10月—2022年10月至南阳市中心医院就诊的67例前列腺癌患者纳入研究,随机分为对照组(34例)和观察组(33例),对照组以筋膜间保留神经腹膜外腹腔镜进行根治性切除,观察组以改良后的经腹膜外全筋膜内腹腔镜根治性切除为主,比较两组患者的并发症、控尿、勃起功能以及手术指标.结果 对照组术后并发症发生率(17.64％)明显高于观察组,组间差异具有统计学意义(P＜0.05);两组患者手术相关指标比较显示组间差异无统计学意义(P＞0.05);观察组术后不同阶段的勃起功能、控尿功能显示正常的人数显著多于对照组,组间差异显著(P＜0.05).结论 对于局限性前列腺癌的治疗,临床可以使用腹腔镜下改良经腹膜外全筋膜内根治性手术进行干预,不仅可以降低整体并发症的发生率,还可以有效恢复患者的勃起功能、控尿功能,临床应用具有安全性和可靠性,值得进行大力推广.
Abstract
Objective To explore the application value of modified transperitoneal total fascial laparoscopic radical resec-tion for localized prostate cancer.Methods A total of 67 patients with prostate cancer who visited to Nanyang Central Hospital from October 2021 to October 2022 were included in the study,and randomly divided into control group(34 ca-ses)and observation group(33 cases).The control group underwent radical resection by interfascia-sparing peritoneal laparoscopic surgery,while the observation group underwent improved radical resection by peritoneal peritoneal peritoneal laparoscopic surgery.Complications,urinary control,erectile function and surgical indexes were compared between the two groups.Results The overall incidence of postoperative complications in the observation group was significantly lower than that of the control group(P＜0.05).There was no significant difference in surgical related indexes between the two groups(P＞0.05).The number of patients in the observation group with normal erectile function and urine control function at different stages was significantly higher than that in the control group(P＜0.05).Conclusion For the treatment of local-ized prostate cancer,laparoscopic modified transperitoneal radical surgery can be used for clinical intervention,which can not only reduce the incidence of overall complications,but also effectively restore the erectile function and urinary control function of patients.The clinical application has safety and reliability,and is worth to be promoted.
