Impact of laparoscopic inferior mesenteric artery ligation position on short-term prognosis in patients with colorectal cancer
Objective To observe the impact of laparoscopic ligation of the inferior mesenteric artery on short-term prognosis in patients with colorectal cancer. Methods Totally 81 patients with colorectal cancer admitted to Zhoukou Central Hospital from January 2021 to November 2023 were selected.Among them,40 patients underwent intraoperative ligation of the inferior mesenteric artery from the abdominal aorta to separate and ligate, defined as the high ligation group;41 patients underwent intraoperative ligation of the inferior mesenteric artery at the level of the left colonic artery branch for separation and ligation, defined as the low ligation group.Collect clinical and surgical data from patients, compare surgical related parameters and postoperative recovery between two groups. Results There was no significant difference in the duration of surgery, number of lymph nodes cleared and metastasized, blood loss, and blood transfusion during hospitalization between the two groups. There was also no significant difference in the time of first postoperative e-vacuation,defecation,fluid feeding, soft feeding, and hospitalization. There were 11 cases(27.50%)of postoperative bladder dysfunction,4 cases(10.00%)of anastomotic leakage, and 14 cases(35.00%)of any complications in the high ligation group, all of which were higher than those in the low ligation group. The postoperative ICIQ and IPSS scores were higher in both groups than those before, and the elevation was more significant in the high ligation group. Conclusion High ligation of the inferior mesenteric artery results in poor preservation of urinary function in patients, leading to an increased incidence of postoperative anastomotic fistula.
