目的 探讨CT脑灌注在急性脑梗死患者早期神经功能评估中的应用价值。方法 选取濮阳市安阳地区医院神经内科于2020年2月1日至2023年2月1日收治的100例急性脑梗死(acute cerebral infarct,ACI)患者作为研究对象,对全部患者进行颅脑CT平扫及CT脑灌注检测,根据患者神经功能是否发生恶化分为恶化组与非恶化组。对两组患者的一般资料、CT脑灌注指标(CBF、CBV、MTT)进行对比,分析CT脑灌注对ACI患者早期神经功能恶化(early neurological deterioration,END)的预测价值。结果 100例ACI患者中,16例(16。00%)发生END;两组患者在性别、年龄、BMI等一般资料上,无显著差异(P>0。05);多因素分析结果表明,MTT为ACI患者发生END的独立影响因素。CBF、CBV、MTT联合预测ACI患者发生END的灵敏度为89。15%、特异度为79。23%。结论 CT脑灌注参数对ACI患者发生END具有较好的预测价值,为提高临床患者的诊治疗效提供了参考。
Application value of CT cerebral perfusion in early neurological function assessment of patients with acute cerebral infarction
Objective To investigate the application value of CT cerebral perfusion in early neurological function evalua-tion of patients with acute cerebral infarction.Methods In the department of neurology of Anyang District Hospital in February 1,2020 to February 1,2023,totally 100 cases of acute cerebral infarction(acute cerebral infarction,ACI to infarct)were used as the object,the brain CT plain scan and CT cerebral perfusion detection were performed on all the patients,Patients were divided into two groups according to whether neurological function deteriorated or not.General data and CT brain perfusion indexes(CBF,CBV,MTT)were compared between the two groups of patients,and the prediction value of CT brain perfusion on early neurological deterioration(END)in ACI patients was analyzed.Results Among 100 patients with ACI,16 cases(16.00%)had END.There were no significant differences in gen-der,age,BMI and other general information between the two groups(P>0.05).The results of multivariate analysis showed that MTT was an independent factor affecting the occurrence of END in ACI patients.The sensitivity and speci-ficity of the combination of CBF,CBV and MTT to predict the occurrence of END in ACI patients were 89.15%and 79.23%respectively.Conclusion CT cerebral perfusion parameters have a good predictive value for the occurrence of END in patients with ACI,which provides a reference for improving the diagnosis and treatment of clinical patients.
