目的 观察全麻术后从不同场所非计划转入ICU的临床特征和结局。方法 回顾性收集2019年4月至2022年4月于河南省人民医院行全麻手术，术后因不良事件非计划转入ICU的患者的病例资料，按照由麻醉后监护治疗室(Post-anesthesia care unit，PACU)直接转入ICU或由PACU顺利转回普通病房后又转入ICU，分为PACU组和病房组。收集两组患者的一般资料、麻醉时间、手术时间、术中是否输血、PACU停留时间、转入ICU的主要不良事件、ICU停留时间、总住院时间和住院死亡率。主要结局为住院死亡率，次要结局为ICU停留时间和总住院时间。结果 转入PACU患者共122 546例，其中689例(0。56％)因不良事件术后非计划转入ICU，445例(0。36％)由PACU转入，244例(0。20％)由普通病房转入。ICU停留时间和总住院时间病房组均显著长于PACU组(P＜0。05)，两组住院死亡率差异无统计学意义(P＞0。05)。结论 全麻手术后非计划转入ICU率较低，ICU停留时间和总住院时间病房组均长于PACU组。
Clinical features and outcomes of unplanned ICU admission from different places after general anesthesia
Objective To observe the clinical features and outcomes of unplanned ICU admission(UIA)after general anes-thesia from different places.Methods The medical data of patients who underwent general anesthesia in Henan Provincial People's Hospital from April 2019 to April 2022 were unplanned transferred to ICUdue to adverse events after surgery were ret-rospectively collected.The patients were divided into PACU group and ward group according to whether they were transferred directly to the ICU from the post-anesthesia care unit(PACU)or successfully transferred back to the general ward from PACU and then transferred to the ICU.General data,anesthesia time,operation time,intraoperative blood transfusion,PACU stay time,major adverse events transferred to ICU,length of ICU stay,total length of stay and in-hospital mortality of the two groups were collected.The primary outcome was in-hospital mortality,and the secondary outcome was length of ICU stay and total length of stay in the hospital.Results A total of 122,546 patients were transferred to PACU,of whiich 689(0.56％)were transferred to ICU unplanned after surgery due to adverse events,445(0.36％)were transferred directly from PACU,and 244(0.20％)were transferred from ward.The length of ICU stay and total length of stay in the ward group were significantly longer than those in the PACU group(P＜0.05),and there was no statistically significant difference in in-hos-pital mortality between the two groups(P＞0.05).Conclusion The ratio of unplanned ICU transfer is low,and the length of ICU stay and total length of stay in the ward group are both longer than those in the PACU group.
