急性前葡萄膜炎疾病严重程度与辅助性T细胞相关因子水平的关系探讨

摘要
目的 探讨急性前葡萄膜炎(acute anterior uveitis,AAU)疾病严重程度与辅助性T细胞(helper T cell,Th)相关细胞因子水平的关系.方法 采用前瞻性病例对照研究设计,收集2021年5月至2022年5月在郑州大学第二附属医院眼科就诊的AAU患者40例,另选取同期体检者30例为对照组,采用ELISA检测受检者外周血中白细胞介素-4(IL-4)、白细胞介素-9(IL-9)、白细胞介素-(IL-17)以及转化生长因子-β1(TGF-β1),比较两组上述指标水平的差异性,并根据EIU评分衡量AAU疾病严重情况,分析AAU患者IL-4、IL-17、IL-9和TGF-β1水平与EIU评分的关系.结果 AAU组患者的IL-4、1L-9、IL-17和TGF-β1水平均较对照组高(P＜0.05).IL-4、IL-9、IL-17以及 TGF-β1 均与 EIU 评分呈正相关(r=0.426、0.544、0.596、0.514,P=0.006、0.000、0.000、0.001).AAU 患者的 EIU 评分中,0～3 分 12 例,4～6 分 15 例,7～9 分 13 例.IL-4 与IL-9呈正相关(r=0.418,P=0.007),IL-4 与 IL-17、TGF-β1 均无明显相关性(r=0.293、0.265,P均＞0.05),IL-9 与 IL-17、TGF-β1 均无明显相关性(r=0.129、0.161,P均＞0.05),IL-17 与 TGF-β1 无明显相关性(r=0.280,P＞0.05).结论 Th相关细胞因子水平变化与AAU的发生发展密切相关,且IL-4与IL-9呈相关性,可能在AAU的发病机制中有着较为重要的作用.
Abstract
Objective To investigate the relationship between the severity of acute anterior uveitis(AAU)and the level of helper T cell(Th)related cytokines.Methods A prospective case-control study design was used.A total of 40 patients with AAU admitted to the ophthalmology department of the Second Affiliated Hospital of Zhengzhou University from May 2021 to May 2022 were collected,and another 30 patients who underwent physical examination during the same period were selected as the control group.The levels of interleukin-4(IL-4),interleukin-9(IL-9),interleukin-17(IL-17)and transforming growth factor-β1(TGF-β1)in peripheral blood of the patients were detected by ELISA,and the differences of the above indexes between the two groups were compared.The severity of AAU disease was measured accord-ing to EIU score,and the relationship between the levels of IL-4,IL-17,IL-9 and TGF-β1 and EIU score in AAU patients was analyzed.Results The levels of IL-4,IL-9,IL-17 and TGF-β1 in AAU group were higher than those in control group(P＜0.05).IL-4,IL-9,IL-17 and TGF-β1 were positively correlated with EIU score(r=0.426,0.544,0.596,0.514,P=0.006,0.000,0.000,0.001).The EIU score of AAU patients was 0-3 in 12 cases,4-6 in 15 cases,and 7-9 in 13 cases.There was a positive correlation between IL-4 and IL-9(r=0.418,P=0.007),but no significant correlation between IL-4 and IL-17 and TGF-β1(r=0.293,0.265,both P＞0.05).There was no significant correlation between IL-9 and IL-17 and TGF-β1(r=0.129,0.161,both P＞0.05),and no significant correlation between IL-17 and TGF-β1(r=0.280,P＞0.05).Conclusion The changes of Th-related cytokines are closely related to the occurrence and development of AAU,and IL-4 and IL-9 exhibita correlation that may play an im-portant role in the pathogenesis of AAU.
