Clinical comparison of preferentially severed pulmonary veins and pulmonary arteries in thoracoscopic lobectomy
Objective To investigate the difference of short-term efficacy between pulmonary vein and pulmonary artery in thoracoscopic lobectomy.Methods A total of 177 patients who underwent thoracoscopic pulmonary lobectomy in General Hospital of Pingmei Shenma group from January 2020 to January 2022 were selected as the objects of this study,and the patients were grouped by envelope method:patients in group A were given priority to have pulmonary artery se-vered during the operation,and patients in group B were given priority to have pulmonary vein severed during the opera-tion.Intraoperative and postoperative surgical indicators and pulmonary residual blood in the two groups were compared,and patients were followed up one year after the operation,and the incidence of complications in the perioperative period and follow-up period were analyzed.Results The postoperative blood loss in group A was smaller than that in group B(P<0.05),but there were no differences in operation time,postoperative hemoglobin decline,number of blood trans-fusions,postoperative drainage time and hospital stay between the two groups(P>0.05).There was no difference in pulmonary residual blood between groups A and B(P>0.05).The pulmonary functions of group A and Group B were decreased 5 days after surgery(P<0.05),but there was no difference between groups before surgery and 5 days after surgery(P>0.05).There was no difference in perioperative and follow-up complications between groups A and B(P>0.05).Conclusion In thoracoscopic lobectomy,the amount of blood loss in the first amputation of pulmonary ar-tery is less than that in the first amputation of pulmonary vein.There is no significant difference in the impact of the two treatment sequences on patients'pulmonary residual blood,postoperative lung function and complications.In clinical op-eration,it is possible to flexibly choose the amputation sequence reasonably.
