目的 探讨高速涡轮手机在下颌低位阻生第三磨牙拔除中的应用,并观察患牙拔除的情况及患者张口受限程度和并发症发生情况。方法 选取2021年8月—2023年8月郑州颐和医院口腔科收治的120例拟接受拔除下颌低位阻生第三磨牙患者,随机分为观察组和对照组各60例。对照组患者行凿骨、劈开牙冠法拔除,观察组患者应用高速涡轮手机磨开牙冠、挺出牙并拔除患牙。比较两组患者术后张口受限程度及术后疼痛程度以及并发症发生情况。结果 对照组拔牙时间[(24。21±3。11)min]长于观察组[(17。08±2。19)min](P<0。05);观察组VAS评分(2。27±0。73)低于对照组患者(3。70±0。66),差异具有统计学意义(P<0。05);应用游标卡尺测量并记录两组患者拔除患牙术后7 d张口受限程度,观察组患者张口受限度显著低于对照组(P<0。05);观察组并发症总发生率(3。33%)低于对照组(P<0。05)。结论 针对下颌低位第三磨牙阻生患者利用高速涡轮手机拔除患牙,可提高手术效率并降低患者疼痛程度,缓解术后张口受限情况,并具有较高的安全性,值得临床推广。
Application of high-speed turbine handpiece in extraction of impacted third molar in lower mandible and its influence on patients'mouth opening limitation
Objective To investigate the application of high-speed turbine handpiece in the extraction of the third mo-lar impacted in the lower mandible,and observe the extraction situation of the affected tooth,the degree of limitation of the patient's mouth opening and the occurrence of complications.Methods A total of 120 patients with low impacted mandibular third molar extraction who were admitted to the Stomatology Department of Zhengzhou Yihe Hospital from Au-gust 2021 to August 2023 were randomly divided into the observation group and the control group,with 60 patients in each group.The control group underwent bone chisel and split crown extraction,while the observation group applied high-speed turbine handpiece to sharpen the crown,extrate the tooth and extract the affected tooth.The degree of post-operative mouth opening limitation,postoperative pain and complications were compared between the two groups.Results The extraction time of control group[(24.21±3.11)min]was longer than that of observation group[(17.08±2.19)min](P<0.05).VAS score of observation group(2.27±0.73)was lower than that of control group(3.70±0.66),and the difference was statistically significant(P<0.05).The mouth opening limitation of pa-tients in the observation group was significantly lower than that in the control group(P<0.05).The total complication rate of observation group(3.33%)was lower than that of control group(P<0.05).Conclution The use of high-speed turbine handpiece to extract the affected teeth in patients with mandibular low third molar obstruction can improve the surgical efficiency and reduce the degree of pain of the patients,alleviate the postoperative mouth opening limitation,and has a high degree of safety,which is worthy of clinical promotion.
