Changes of myocardial injury markers in patients with myocardial infarction and relationship with prognosis
Objective To analyze the changes of myocardial injury markers in patients with myocardial infarction(MI),and to explore the relationship with prognosis of patients.Methods Totally 119 patients with MI(MI group)admitted to the hospital from January 2020 to December 2022 and 150 physical examination people(control group)during the same pe-riod were selected.According to the time from onset to admission,MI patients were divided into 0-<4 h group(n=33),4-<8 h group(n=47)and 8-12 h group(n=39).The patients were followed up for 6 months and were divided into poor prognosis group and good prognosis group according to whether MACE occurred.The levels of serum Myo,cTnI,cT-nT,CK and CK-MB were detected.logistic regression analysis was used to analyze the risk factors affecting the poor prognosis in MI patients.Receiver operating characteristic curve(ROC)was drawn to evaluate the predictive value of Myo,cTnI,cTnT,CK and CK-MB on poor prognosis in MI patients.Results The levels of serum Myo,cTnI,cTnT,CK and CK-MB in MI group were significantly higher than those in control group(P<0.05).There were statistically sig-nificant differences in the levels of serum Myo,cTnI,cTnT,CK and CK-MB in patients with MI at different onset times(P<0.05).Multivariate logistic regression analysis showed that hyperlipidemia,Killip grade Ⅲ-Ⅳ and increased serum levels of Myo,cTnI,cTnT,CK and CK-MB were independent risk factors for poor prognosis in patients with MI(P<0.05).The AUCs of Myo,cTnI,cTnT,CK,CK-MB and their combination in predicting poor prognosis in MI pa-tients were 0.711,0.739,0.817,0.727,0.835 and 0.956 respectively.Conclusion The levels of serum Myo,cTnI,cTnT,CK and CK-MB in patients with MI are significantly increased,and they are independent risk factors for poor prognosis in patients with MI,which can be used as important indicators to predict the prognosis of patients.
