Study on relationship between changes in thyroid globulin levels and therapeutic efficacy in patients with differentiated thyroid cancer after 131I treatment
Objective Exploring the relationship between changes in thyroid globulin levels and therapeutic efficacy in pa-tients with differentiated thyroid cancer after 131I treatment.Methods A retrospective analysis was conducted on clinical data of 92 patients with differentiated thyroid cancer who received 131I treatment in Kaifeng Central Hospital from January 2020 to September 2023.According to the efficacy status after 6 months of treatment,they were divided into a remission group of 70 cases and an unrelieved group of 22 cases.The application of multiple logistic regression analysis to explore the influencing factors of the efficacy of 131I treatment in differentiated thyroid cancer patients,and the value of predicting the efficacy of 131I treatment in differentiated thyroid cancer patients through receiver operating characteristics(ROC)curve analysis of changes in thyroid globulin levels.Results Univariate analysis showed that there was no statistically significant difference between the two groups in terms of age,gender,T stage,M stage,and thyroid invasion(P>0.05),but there was a statistically signifi-cant difference in tumor diameter,N stage,preablation stimulated Tg and non-stimulated Tg between the two groups(P<0.05).Binary logistic regression analysis showed that N stage(N1a-b stage),ps Tg level(>10.0 μg/L),and ns-Tg level(>1.0 μg/L)were all influencing factors for the effi-cacy of 131I treatment in patients with differentiated thyroid cancer(P<0.05).ROC curve analysis showed that the AUC of ps Tg and ns-Tg predicting the efficacy of 131I treatment in differentiated thyroid cancer patients were 0.768 and 0.645,respec-tively;the sensitivity is 59.1%and 97.1%respectively;the specificity was 59.1%and 70.0%,respectively.The ps Tg and ns-Tg conditions were included in the binary logistic regression model to calculate the combined data[combined data=Ps Tg condition+(1.660/3.314)xns-Tg condition].The AUC of the combined prediction data for predicting the efficacy of 131I treatment in differentiated thyroid cancer patients was 0.804;sensitivity was 77.3%and the specificity was 75.7%.Conclusion The changes in thyroid globulin levels before and after treatment can serve as an effective indicator for predicting the efficacy of 131I treatment in patients with differentiated thyroid cancer,and the combined predictive value is better.
