Analysis of the aetiological characteristics and susceptibility factors for development of pulmonary infections in hospitalised patients with COPD
Objective To analyze the pathogenic characteristics of pulmonary infection in chronic obstructive pulmonary disease(COPD)inpatients,and to analyze its susceptible factors by lasso method.Methods Totally 218 COPD inpa-tients admitted to Zhoukou disease prevention control center college disease hospital between March 2021 and March 2023 were included in this study,and their clinical data were retrospectively analysed,assessed for the occurrence of lung infections based on bacterial culture results and CT imaging manifestations,and classified into an infected group(n=93)and a non-infected group(n=125),and analysed for their susceptibility factors by unifactorial and multifac-torial methods.Results The incidence of pulmonary infections in patients with COPD in this study was 42.66%(93/218),and a total of 87 pathogenic organisms were detected by pathogen examination,of which 56 strains of gram-neg-ative bacteria were detected,with Klebsiella pneumoniae predominantly(24.14%).Bed-ridden time ≥1 month,age≥60 years,history of mechanical ventilation,number of antimicrobials used ≥2,use of ventilator,indwelling catheter,comorbid diabetes mellitus,and history of long-term hormone use were independent risk factors affecting the complica-tion of pulmonary infection in patients with COPD(all P<0.05).Conclusion Klebsiella pneumoniae is the main pathogen causing lung infections in hospitalised patients with COPD,and its susceptibility factors include comorbid dia-betes mellitus,age over 60 years,bed-ridden time of not less than 1 month,indwelling catheter,use of ventilator,his-tory of mechanical ventilation,history of long term hormone use,and use of antimicrobial drugs in the number of not less than two,and appropriate interventions should be formulated promptly to address the above risk factors.
