Application of DE-MRI in the diagnosis of primary dilated cardiomyopathy and analysis of imaging characteristics
Objective To evaluate the diagnostic value of delayed enhanced magnetic resonance imaging(DE-MRI)in idiopathic dilated cardiomyopathy(IDCM),analyze its imaging characteristics,and provide theoretical basis for the ap-plication of DE-MRI.Methods A total of 127 patients with IDCM to be diagnosed in Xinxiang Central Hospital from January 2020 to December 2022 were selected.Coronary lesions were excluded by computed tomography angiography(CTA).The image characteristics of IDCM in DE-MRI were analyzed,and the sensitivity,specificity,accuracy,pos-itive predictive value,and negative predictive value of DE-MRI in the diagnosis of IDCM were explored.Cardiac mag-netic resonance(CMR)parameters,cardiac troponin 1(cTnⅠ)and brain natriuretic peptide(BNP)in patients with dif-ferent late gadolinium enhanced(LGE)outcomes were compared.Results A total of 72 patients in the IDCM group were LGE positive,including 7 cases of transmural delayed enhancement,60 cases of intramural delayed enhancement,and 5 cases of subendometrial delayed enhancement;the LGE positivity rate of 84.71%in the IDCM group was signifi-cantly higher than that of 7.14%in the non IDCM group.The sensitivity,specificity,accuracy,positive predictive value,and negative predictive value of DE-MRI in diagnosing IDCM were 82.35%,95.24%,86.61%,97.22%,and 72.73%,respectively,with Kappa of 0.720.LVEF of LGE positive patients is lower than that of LGE negative pa-tients,while Left ventricular end diastolic volume index(LVEDVi),end systolic volume index(LVESVi),cardiac tro-ponin Ⅰ(CTnⅠ),and B-type natriuretic peptide(BNP)are higher than those of LGE negative patients.Conclusion IDCM patients are mainly characterized by delayed enhancement between walls in DE-MRI,and DE-MRI has good diagnostic efficacy for IDCM,which can be used for clinical diagnosis.
