Prediction and evaluation value of dynamic change of vitamin D and development degree of pulmonary infection and inflammation in children
Objective To explore the correlation between vitamin D level and inflammation indicators of pediatric pulmo-nary infection and to analyze the value of vitamin D in diagnosis and evaluation of pediatric pulmonary infection.Methods A total of 200 children with pulmonary infection admitted to the Xuchang Central Hospital from January 2022 to December 2022 were selected as research objects.According to the results of chest radiographs or chest CT,they were divided into mild group(100 cases,not up to the standard of severe,with stable vital signs)and moderate and severe group(100 cases,reached moderate to severe standard,respiratory rate>30 times/min,lesions involving bilateral or multiple lung lobes).Serum 25 hydroxyvitamin D3[25-(OH)D3]was determined by enzymed-linked immunosorbent assay,and the difference of 25-(OH)D3 between the two groups was compared.The correlation between 25-(OH)D3 and inflammatory indicators of pulmonary infection in children and its application value in disease prediction and diagno-sis were discussed.Results The serum 25-(OH)D3 value of mild group was higher than that of moderate and severe group 48h after admission,and the values of CRP,IL-6,TNF-α,PCT and WBC were lower than those of moderate and severe group,with statistical significance(P<0.05).Pearson correlation analysis showed that 25-(OH)D3 was negatively correlated with CRP,IL-6,TNF-α,PCT and WBC(r=-0.325,-0.526,-0.622,-0.725,-0.688,P<0.05).logistic regression analysis showed that high expression of 25-(OH)D3 was a protective factor for moderate and severe lung infection,and high expression of CRP,IL-6,TNF-α,PCT and WBC was a risk factor for moderate and severe lung infection(P<0.05).Conclusion High expression of vitamin D is a protective factor for moderate and severe pulmonary infection.Serum 25-(OH)D3 is correlated with CRP,IL-6,TNF-α,PCT and WBC.Serum 25-(OH)D3 detection in children with pulmonary infection can reflect the level of vitamin D and the de-gree of inflammation,and provide an important reference for the clinical diagnosis and evaluation of the degree of pulmo-nary infection in children.
