Application of serum MMP-9 and LPA2 levels detection in early detection of large vessel lesions in ischemic cerebrovascular disease and prognosis evaluation
Objective To explore and analyze the application of serum MMP-9 and LPA2 levels detection in early de-tection of large vessel lesions in ischemic cerebrovascular disease and prognosis evaluation.Methods Totally 140 pa-tients with ischemic cerebrovascular disease from January 2020 to December 2023 in our hospital were selected as the re-search subjects,including 80 patients with cerebral infarction as the cerebral infarction group;60 patients with transient cerebral ischemia were included in the TIA group;And 30 healthy individuals were selected as the control group.Ana-lyze and compare the serum MMP-9 concentration results of three groups of patients after 12 hours of admission;Ana-lyze the relationship between serum MMP-9 levels,neurological dysfunction,and stroke volume in patients with cere-bral infarction after 48 hours;Provide the same treatment to patients in the cerebral infarction group and TIA group,and analyze and compare the changes in serum LPA2 concentration of the three groups of patients at 3 days,2 weeks,and 4 weeks of admission;Compare the MAS scores of patients with cerebral infarction between the low LPA2 population and the high LPA2 population after 4 weeks of treatment;Based on the above comparison,determine the relationship between early serum MMP-9 and LPA2 levels in ischemic cerebrovascular disease and the disease itself and prognosis.Results The concentration of MMP-9 within 12 hours in patients with acute cerebral infarction was(143.6±21.3)ng/mL,in TIA patients it was(118.6±22.3)ng/mL,and in the control group it was(56.6±20.5)ng/mL;The con-centration of MMP-9 in the patient group at 12 hours was significantly higher than that in the control group(P<0.05);The level of MMP-9 in the cerebral infarction group was significantly higher than that in the TIA group,and there was a significant difference between the two(P<0.05).The serum MMP-9 concentration at 48 hours in the cerebral infarction group was positively correlated with the CT scan volume of cerebral infarction(r=0.657,P<0.01)and positively correlated with the NIHSS neurological function score(14.5±83)(r=0.787,P<0.01).At 3 days of admission,the serum LPA2 concentration in the stroke group was(9.65±2.77)µmol/L,while in the TIA group it was(7.52±2.23)μmol/L.In the control group,the LPA2 concentration was(20.2±1.07)µmol/L.The LPA2 concen-tration in the patient group was significantly higher than that in the control group at 3 days(P<0.01);After 4 weeks of treatment,there was no significant difference in the pre treatment MAS scores between the low LPA2 group and the high LPA2 group of patients with cerebral infarction.The difference between the MAS score and the improvement score after treatment is statistically significant.Conclusion Patients with ischemic cerebrovascular disease have elevated levels of serum MMP-9 and LPA2 in the early stages,which can serve as diagnostic criteria for ischemic cerebrovascular dis-ease;Early levels of MMP-9 are positively correlated with neurological damage and cerebral infarction volume,while early levels of LPA2 are negatively correlated with patient prognosis.Therefore,early serum levels of MMP-9 and LPA2 can also be used as indicators for evaluating the prognosis of ischemic cerebrovascular disease.
