目的 分析高亲和型酪氨酸蛋白激酶类受体(tyrosine protein kinase-like receptor,TrkA)、上皮型钙黏蛋白(epithelial calreticulin,E-cadherin)与鼻腔鼻窦腺样囊性癌病理、预后的关系。方法 回顾性收集2019年1月—2021年7月郑州大学第一附属医院收治鼻腔鼻窦腺样囊性癌患者样本68例。用免疫组化SP法检测鼻腔鼻窦腺样囊性癌患者组织标本中TrkA、E-cadherin的表达,分析鼻腔鼻窦腺样囊性癌患者的TrkA、E-cadherin表达及与其病理特征的关系。进行随访,用寿命表法计算鼻腔鼻窦腺样囊性癌患者生存状况。用Kaplan-Meir生存曲线分析鼻腔鼻窦腺样囊性癌患者组织中TrkA、E-cadherin的表达与其生存率的关系;用Cox比例风险模型分析影响鼻腔鼻窦腺样囊性癌患者总生存期的危险因素。结果 68例鼻腔鼻窦腺样囊性癌患者标本中,TrkA表达阳性率为60例(88。23%),E-cadherin表达阳性率为13例(19。11%)。Ⅲ期~Ⅳ期、实质型、局部有复发者的TrkA高表达占比高于Ⅰ期~Ⅱ期、筛状型/管状型、局部无复发者,差异具有统计学意义(P<0。05);Ⅲ期~Ⅳ期、实质型、局部复发、淋巴结侵犯者的E-cadherin低表达占比高于Ⅰ期~Ⅱ期、筛状型/管状型、局部无复发者,差异具有统计学意义(P<0。05)。经随访了解,18例死亡,1年、2年生存率分别为88。23%、73。52%。经Kaplan-Meir生存曲线得知,TrkA高表达2年生存率为66。67%,低于TrkA低表达组90。00%,差异具有统计学意义(P<0。05);E-cadherin低表达2年生存率为72。13%,低于E-cadherin高表达组的85。71%,差异具有统计学意义(P<0。05)。结论 鼻腔鼻窦腺样囊性癌组织中TrkA主要呈高表达,E-cadherin主要呈低表达,两者在鼻腔鼻窦腺样囊性癌发生、发展及预后具有一定作用。进一步探究TrkA和E-cadherin在该癌症中的具体功能和作用机制,对于临床开发新的治疗策略及准确评估患者预后具有重要意义,可为相关治疗提供坚实的理论依据。
Relationship between high-affinity tyrosine protein kinase-like receptors,epithelial calreticulin and pathology and prognosis of adenoid cystic carcinoma of nasal sinuses
Objective To analyse the relationship between high-affinity tyrosine protein kinase-like receptor(TrkA),epithelial calreticulin(E-cadherin)and the pathology and prognosis of adenoid cystic carcinoma of the nasal sinuses.Methods A total of 68 patients with adenoid cystic carcinoma of the nasal sinuses admitted to the First Affiliated Hospital of Zhengzhou University from January 2019 to July 2021 were retrospectively collected.The expression of TrkA and E-cad-herin in the tissue specimens of patients with adenoid cystic carcinoma of the nasal sinuses was detected by immunohisto-chemical SP method,and the expression of TrkA and E-cadherin in patients with adenoid cystic carcinoma of the nasal sinuses and the relationship with their pathological features were analysed.Follow-up visits were performed,and the sur-vival status of patients with adenoid cystic carcinoma of the nasal sinuses was calculated using the life table method.The re-lationship between the expression of TrkA and E-cadherin in the tissues of patients with adenoid cystic carcinoma of the nasal sinuses and their survival was analysed using Kaplan-Meir survival curves;risk factors affecting the overall survival of patients with adenoid cystic carcinoma of the nasal sinuses were analysed using Cox proportional risk models.Results Of the 68 specimens from patients with adenoid cystic carcinoma of the nasal sinuses,60(88.23%)were positive for TrkA expression and 13(19.11%)for E-cadherin expression.The percentage of high expression of TrkA was higher in those with stage Ⅲ-Ⅳ,parenchymal type,and local recurrence than in those with stage Ⅰ~Ⅱ,sieve/tubular type,and local no recurrence,and the difference was statistically significant(P<0.05);the percentage of low expression of E-cadherin was higher in those with stage Ⅲ-Ⅳ,parenchymal type,local recurrence,and lymph node invasion than in those with stage Ⅰ~Ⅱ,sieve/tubular type,and local no recurrence,and the difference was was statistically significant(P<0.05).On follow-up,18 deaths were known,with 1-year and 2-year survival rates of 88.23%and 73.52%,respectively.The Kaplan-Meir survival curve showed that the 2-year survival rate of TrkA high-expression was 66.67%,which was low-er than that of 90.00%in the TrkA low-expression group,and the difference was statistically significant(P<0.05);the 2-year survival rate of E-cadherin low-expression was 72.13%,which was lower than that of 85.71%,the difference was statistically significant(P<0.05).Conclusion TrkA and E-cadherin are highly expressed and lowly expressed in the tissues of adenoid cystic carcinoma of the nasal sinus,and both of them play a certain role in the occurrence,develop-ment and prognosis of adenoid cystic carcinoma of nasal cavity and sinuses.Further investigation of the specific function and mechanism of TrkA and E-cadherin in this cancer is of great significance for clinical development of new treatment strategies and accurate assessment of patient prognosis,and provides a solid theoretical basis for related treatment.
