Analysis of spinal cord imaging,neuropsychology and cerebrospinal fluid biomarkers in patients with NMOSD and multiple sclerosis
Objective Analyze differences between clinical characteristics of patients with neuromyelitis optic spectrum disease(NMOSD)and multiple sclerosis(MS).Methods A total of 90 patients with NMOSD and 45 patients with MS in the First Affiliated Hospital of Henan University were enrolled as NMOSD group and MS group between January 2021 and June 2023,respectively.All patients underwent scan of skull and spinal cord by magnetic resonance scanners,and chan-ges of brain volume were evaluated by voxel-based morphometry(VBM).The physical function and disability were eval-uated by expanded disability status scale(EDSS).The depression and anxiety were evaluated by Hamilton depression scale-17 item(HAMD17)and Hamilton anxiety scale(HAMA).The verbal and behavioral abilities were evaluated by verbal fluency test(VFT),rey complex figure test and rec-ognition trial(RCFT)and the Chinese version of the stroop colour-word test(Stroop).The oligoclonal bands(OBs)in cerebrospinal fluid were detected by electrofocusing electrophoresis and immunofixation staining.The levels of IgA,IgM and IgG in cerebrospinal fluid were detected by immunoturbidimetry.The myelin oligodendrocyte glycoprotein(MOG)an-tibody,myelin basic protein(MBP)antibody and aquaporin 4(AQP4)antibody were detected by immunofluorescence as-say.The antinuclear antibody(ANA)in cerebrospinal fluid was detected by indirect immunofluorescence assay.The lev-els of B lymphocyte subsets in cerebrospinal fluid were detected by flow cytometry.Results The scores of EDSS,HAMA and HAMD-17 in NMOSD group were higher than those in MS group,while VEF score was lower than that in MS group(P<0.05).The positive rate of OBs in NMOSD group was lower than that in MS group,while positive rate of AQP4 anti-body was higher than that in MS group(P<0.05).The proportion of CD38high in cerebrospinal fluid in NMOSD group was higher than that in MS group(P<0.05).There was no significant difference in volume of skull lesions on T2 imaging between NMOSD group and MS group[(6.93±2.11)mL vs(6.59±2.03)mL,P>0.05].Conclusion Compared with MS patients,NMOSD patients have obvious cognitive impairment such as semantic fluency,and the positive rate of OBs and AQP4 level in cerebrospinal fluid are higher.
