Clinical observation of OGTT characteristics,prevalence of gestational diabetes mellitus and pregnancy outcomes in twin pregnant women with different pre pregnancy BMI
Objective o explore the oral glucose tolerance test of twin and single pregnant women with different pre-pregnancy body mass index(BMI).Differences in the characteristics of OGTT and the prevalence of gestational diabetes mellitus(GDM),and their effects on pregnancy outcomes were analyzed to provide theoretical support for formulating targeted interventions,reducing the risk of GDM in pregnant women with twins and improving pregnancy outcomes.Methods Totally 132 pregnant women with singleton and twin pregnancies in the Kaifeng Obstetrics and Gynecology Hospital from January 2022 to December 2023 were selected and divided into lean/normal group and overweight/obese group according to BMI.OGTT examination and GDM diagnosis were carried out according to the guidelines.The OGTT 0 h,OGTT1 h,OGTT 2 h blood glucose levels,GDM incidence and pregnancy outcomes of pregnant women with differ-ent pre pregnancy BMI were compared.Results The incidence of GDM in twin pregnant women was 24.24%,which was significantly higher than that in single pregnant women 14.39%(P<0.05).The OGTT 0 h,OGTT 1 h and OGTT 2 h blood glucose levels in twin pregnant women were significantly higher than that in single pregnant women(P<0.05).A total of 190 patients had BMI as emaciated/normal,including 95 singleton and 95 twin.There were 74 overweight/obese patients,of which 37 were single and 37 were twin.The prevalence of GDM in overweight/obese women in single and twin pregnant women was higher than that in emaciated/normal women(P<0.05).The prevalence of GDM in overweight/obese women in twin pregnant women was higher than that in single pregnant women(P<0.05).Fasting,OGTT 1 h and OGTT 2 h blood glucose levels of overweight/obese pregnant women were higher than those of e-maciated/normal pregnant women(P<0.05),and OGTT 1 h and OGTT 2 h blood glucose levels of twin pregnant women were significantly higher than those of single pregnant women(P<0.05).The rates of hypertension,pre-ec-lampsia and fetal growth restriction(FGR)during pregnancy in overweight/obese twins were higher than those in emaci-ated/normal women,and the rates of anemia were lower than those in emaciated/normal women(P<0.05).Conclusion Twin pregnant women should pay attention to pre pregnancy BMI.The OGTT level of overweight/obese twin pregnant women is relatively high,and the risk of GDM and adverse pregnancy outcomes is high.
