目的 系统评价双柏膏治疗急性踝关节扭伤的临床疗效,为临床合理应用提供循证治疗依据.方法 通过计算机检索国内外中英文数据库,主要包括国家知识基础设施数据库(CNKI)、万方数据库、维普中文数据库(VIP)、中国医学文献数据库(CBM)、PubMed、Web of Science、Cochrane图书馆等数据库,收集自建库以来至2024年3月双柏膏外用与其他治疗方式治疗急性踝关节扭伤的随机对照试验(RCT),制定纳入标准和排除标准后,通过Endnote筛选出符合相应标准的文献,评价纳入文献的质量并提取数据,进行汇总后采用Revman5.3软件对数据进行Meta分析.结果 共有4篇文献符合纳入标准,纳入患者共260例.Meta分析结果显示双柏膏外用治疗急性踝关节扭伤后,与对照组相比,能够明显降低VAS评分[MD=-0.72,95％CI:-1.14～-0.31,P=0.000 7],能够显著改善踝关节功能AOFAS评分[MD=8.29,95％CI:5.78～10.80,P＜0.000 01],在总体有效率评价方面,双柏膏与其他治疗方式相比,差异无统计学意义[RR=1.22,95％CI:0.96～1.31,P=0.15].结论 双柏膏外用能够明显改善急性踝关节扭伤患者局部的疼痛,恢复踝关节活动功能,但纳入文献总体有效率与其他治疗方式无明显差别.
Meta-analysis of clinical efficacy of Shuangbai ointment in treatment of acute ankle sprain
Objective To systematically evaluate the clinical therapeutic effects of Shuangbai ointment in the treatment of acute ankle sprain,and provide evidence-based treatment basis for rational clinical application.Methods Ran-domized controlled trials(RCTs)investigating the efficacy of Shuangbai ointment and various treatment modalities for acute ankle sprain were systematically retrieved from databases including the national knowledge infrastructure database(CNKI),Wanfang database,VIP Chinese database,Chinese medical literature database(CBM),PubMed,Web of Science,Cochrane Library,etc.,spanning from the inception of the databases to March 2024.Following the establish-ment of inclusion and exclusion criteria,the literature meeting these criteria underwent screening via EndNote,evalua-tion of the quality of the included literature,and extraction of data.Subsequently,the data were summarized and sub-jected to meta-analysis using Revman 5.3 software.Results A total of 4 literatures met the inclusion criteria,and a total of 260 patients were included.Meta-analysis results showed that after the treatment of acute ankle sprain with Shuangbai ointment,compared with the control group,it could significantly reduce the VAS score[MD=-0.72,95％CI:-1.14 to-0.31,P=0.0007],and significantly improve the ankle function AOFAS score[MD=8.29,95％CI:5.78 to 11.80,P＜0.00001].In terms of the overall effectiveness evaluation,Significant difference between Shuangbai ointment and other treatment methods were not observed[RR=1.22,95％CI:0.96 to 1.31,P=0.15].Conclusion Shuangbai ointment can significantly improve the local pain of patients with acute ankle sprain and re-store the ankle function,but there is no significant difference in the overall effectiveness of the included literatures and other treatment methods.
