Prognostic significance of serum C-reactive protein to albumin ratio in elderly patients with newly diagnosed acute myeloid leukemia
Objective To evaluate the prognostic significance of serum C-reactive protein(CRP)to albumin(ALB)ra-tio(CAR)in elderly patients with newly diagnosed acute myeloid leukemia(AML).It provides useful reference for fur-ther exploring the pathogenesis of AML and finding new therapeutic targets.Methods Totally 194 elderly patients with AML treated from July 2018 to December 2023 were selected.The receiver operating characteristic(ROC)curve was used to determine the critical value of car.The clinical characteristics,prognosis,and the correlation with overall survival(OS)and event free survival(EFS)of patients with different levels of car were compared.Results There were 124 patients(63.92%)achieved complete response(CR)after two cycles of induction chemotherapy,with a CAR cutoff of 1.015 and an AUC of 0.690(95%CI:0.525-0.699,P<0.001),including 86 patients in the high CAR group and 108 patients in the low CAR group.The proportion of patients with age,CRP level,ferritin level,bone marrow original cell percentage and ELN risk stratification was lower in the low CAR group than those in the high CAR group,and the proportion of ALB level and CR1 was higher than those in the high CAR group.60 cases(55.56%)and 107 cases(99.07%)of low CAR group were CD15 positive,which were higher than 31 cases(36.05%)and 80 cases(93.02%)of high CAR group.The OS(x2=5.133,P<0.001)and EFS(x2=4.362,P<0.001)of patients with high CAR were lower than those with low CAR.WBC ≥ 100 x 109/L,LDH ≥ 245U/L and high CAR were risk factors for OS and EFS in AML patients.Conclusion In newly diagnosed older patients with AML,high car is a poor prognostic factor for 2-year OS and EFS.
