目的 探讨人附睾蛋白4(human epitope protein 4,HE4)、B淋巴细胞瘤-2基因(B lymphoblastoma-2 gene,Bcl-2)、增殖细胞核相关抗原(nuclear-associated antigen,Ki-67)、肿瘤蛋白 p53(tumor protein p53,P53)在卵巢癌患者组织中的表达水平,并分析上述指标与卵巢癌患者病理特征的关系,为该病的诊断及预后评估提供参考依据。方法 选取2021年11月-2024年2月南阳市中心医院收治的卵巢癌患者136例作为研究组,另外选取同期该院收治的卵巢良性肿瘤患者145例作为对照组。手术获取研究组和对照组患者的卵巢组织,利用免疫组织化学法检测卵巢组织中HE4、Bcl-2、Ki-67、P53表达情况。比较两组组织中HE4、Bcl-2、Ki-67、P53表达阳性率,分析卵巢癌患者组织中HE4、Bcl-2、Ki-67、P53阳性表达率与病理特征的关系,Pearson相关性分析卵巢癌患者组织中HE4、Bcl-2、Ki-67、P53表达之间的相关性。结果 研究组组织中HE4、Bcl-2、Ki-67、P53 表达阳性率分别为 61。76%、67。65%、74。26%、82。35%,高于对照组的 29。66%、15。86%、13。79%、11。72%,差异有统计学意义(P<0。05)。国际妇产科联盟(federation international of gynecology and obstetr,FIGO)分期为Ⅲ期的卵巢癌患者组织中HE4、Bcl-2、Ki-67、P53阳性表达率高于FIGO分期为Ⅱ期、I期者,差异有统计学意义(P<0。05),Ⅱ期高于Ⅰ期,差异有统计学意义(P<0。05);分化程度为低分化、有淋巴结转移的卵巢癌患者组织中HE4、Bcl-2、Ki-67、P53蛋白表达阳性率高于分化程度为中高分化、无淋巴结转移者,差异有统计学意义(P<0。05)。Pearson相关性分析结果显示,卵巢癌患者组织中HE4与Bcl-2、Ki-67、P53表达呈正相关(r=0。533、0。497、0。488,P<0。05);Bcl-2 与 Ki-67、P53 表达呈正相关(r=0。509、0。521,P<0。05);Ki-67 与P53表达呈正相关(r=0。495,P<0。05)。结论 卵巢癌患者组织中HE4、Bcl-2、Ki-67、P53呈高表达,四者表达水平具有一定的关联性,可能协同参与卵巢癌疾病的进展过程。
Study on relationship between tissue HE4,Bcl-2,Ki-67,and P53 expression levels and their pathological features in patients with ovarian cancer
Objective To explore the expression levels of human epitope protein 4(HE4),B lymphoblastoma-2 gene(Bcl-2),nuclear-associated antigen(Ki-67),tumor protein p53(P53)in the tissues of ovarian cancer patients,and to analyze the relationship between the above indicators and the pathological characteristics of ovarian cancer patients,in order to provide a reference basis for the diagnosis and prognosis assessment of this disease.Methods Totally 136 pa-tients with ovarian cancer admitted to Nanyang central Hospital from November 2021 to February 2024 were selected as the study group,and another 145 patients with benign ovarian tumors admitted to the hospital during the same period were selected as the control group.The ovarian tissues of the patients in the study group and the control group were surgically obtained,and the expression of HE4,Bcl-2,Ki-67 and P53 in the ovarian tissues were detected by immunohistochem-istry.The positive rates of HE4,Bcl-2,Ki-67 and P53 expression in the tissues of the two groups were compared,the relationship between the positive rates of HE4,Bcl-2,Ki-67 and P53 expression in the tissues of patients with ovarian cancer and pathological features were analyzed,and the correlation between the expression of HE4,Bcl-2,Ki-67 and P53 in the tissues of patients with ovarian cancer were analyzed using Pearson correlation analysis.Results The positive expression rates of HE4,Bcl-2,Ki-67 and P53 in the tissues of the study group were 61.76%,67.65%,74.26%and 82.35%,respectively,which were higher than those of the control group,which were 29.66%,15.86%,13.79%and 11.72%,respectively(P<0.05).The positive expression rate of HE4,Bcl-2,Ki-67,P53 positive expression rate was higher than those with federation international of gynecology and obstetr(FIGO)stage Ⅱ and Ⅰ(P<0.05),and stageⅡ was higher than those with stage Ⅰ(P<0.05);the positive rate of HE4,Bcl-2,Ki-67,P53 protein expression in the tissues of patients with ovarian cancer of the degree of differentiation of low-differentiation and lymph node metastasis were higher than those with the degree of differentiation of medium-high differentiation and no lymph node metastasis(P<0.05).Pearson correlation analysis showed that HE4 was positively correlated with the expression of Bcl-2,Ki-67,and P53 in the tissues of ovarian cancer patients(r=0.533,0.497,and 0.488,P<0.05);and that Bel-2 was pos-itively correlated with the expression of Ki-67,and P53(r=0.509,0.521,P<0.05);Ki-67 was positively correlated with P53 expression(r=0.495,P<0.05).Conclusion HE4,Bcl-2,Ki-67,and P53 were highly expressed in the tissues of patients with ovarian cancer,and the expression levels of the four were correlated,which might be synergistically involved in the progression of ovarian cancer disease.
