Effect of sarcopenia on endoscopic submucosal dissection for early gastric cancer
Objective To explore the short-term and long-term efficacy of endoscopic submucosal dissection(ESD)in patients with gastric cancer(GC)combined with sarcopenia.Methods A total of 273 patients with early gastric cancer(GC)who underwent endoscopic submucosal dissection(ESD)in the Second People's Hospital of Nanyang from February 2019 to July 2023 were selected and divided into sarcopenia group and non-sarcopenia group according to whether they were complicated with sarcopenia.Propensity Score Matching was applied.PSM method was used to match the two groups 1∶1 to analyze the effects of sarcopenia on the short and long term outcomes of early GC patients after ESD surgery.Results ① Comparison of age,gender,proportion of hyperlipidemia and diabetes in complications,lymphatic invasion,and intraoperative blood loss between the two groups before PSM treatment(P<0.05).After 1∶1 matching with PSM treatment,there was no statistical significance in the comparison of various indexes between the two groups(P>0.05).②The short-term outcome data of early GC patients after ESD(including four sub indicators such as infection,gastroe-sophageal reflux,gastric bleeding,and gastric perforation),whether before or after PSM,there was no significant differ-ence between sarcopenia group and non-sarcopenia group(P>0.05).③Compared with the data before and after PSM,the survival of patients with sarcopenia was significantly worse than that of non sarcopenia patients.Logrank test showed significant difference(P<0.05).Conclusion The survival rate of patients with sarcopenia after ESD is significantly shorter than that of non sarcopenia patients;Preoperative screening for sarcopenia is crucial for patients.
