Efficacy and short-term prognosis of Q-method traction technique during ESD in treating early esophageal cancer
Objective To investigate the efficacy and prognosis of Q-method traction technique during endoscopic sub-mucosal dissection(ESD)in the treatment of early esophageal cancer.Methods The data of 102 patients with early e-sophageal cancer who received surgical treatment in the hospital were retrospectively analyzed from April 2020 to Decem-ber 2022.According to the different surgical methods,the patients were divided into ESD group and Q-ESD group.The ESD group was treated with conventional ESD,whereas the Q-ESD group adopted Q-method traction technique-assisted ESD.The propensity score matching method was used to balance the confounding bias caused by the differences in baseline characteristics between the two groups.Finally,30 pairs of patients with comparable baseline data were ob-tained.The surgical related indicators,lesion resection effect and postoperative complications were compared.The pa-tients were followed up for 1 year,and the recurrence and survival status were compared.Results The peeling speed and peeling area in Q-ESD group were significantly higher than those in ESD group(P<0.05),and the surgical time,intraoperative blood loss and intraoperative submucosal injection volume were significantly shorter or lower than those in ESD group(P<0.05),but there was no statistical significance in hospital stay between the two groups(P>0.05).The one-time en bloc resection rate and curative resection rate in Q-ESD group were significantly higher(P<0.05),while the total incidence rate of postoperative complications was significantly lower than that in ESD group(P<0.05).At 1 year of follow-up after surgery,there were no deaths in the two groups,and there were no statistical differences in recurrence rates at 6 months and 1 year after surgery(P>0.05).According to K-M survival curve analysis,the progression-free survival time within 1 year showed no statistical difference between groups(P>0.05).Conclusion Q-method traction technique-assisted ESD has the advantages of high efficiency,safety and few complications in the treatment of early e-sophageal cancer,and it has good short-term prognosis.
