数字化3D打印导航模板在髌骨成形术中的应用研究

目的 分析数字化3D打印导航模板在髌骨成形术中的应用效果。方法 选取2022年6月至2023年6月89例在河南大学第一附属医院行人工全膝关节表面置换术(total knee arthroplasty,TKA)治疗的膝关节骨关节炎(knee osteoarthritis,OA)患者,依照随机数字表法分为两组,对照组(44例)采用传统髌骨成形术,观察组(45例)采用数字化3D打印导航模板辅助下的髌骨成形术,比较两组不同时间点(术前、术后1个月、3个月、6个月)膝前疼痛程度(视觉VAS疼痛评分表)、术前及术后6个月影像学指标(髌骨倾斜角、髌骨移位距离、髌韧带比值、胫骨角)、髌骨功能(Feller髌骨评分)、膝关节功能[美国膝关节协会评分(KSS评分)、牛津大学膝关节评分(OKS评分)]。结果 两组不同时间、组间、时间×组间作用下膝前疼痛程度比较,差异具有统计学意义(P<0。05),随着时间推移,两组膝前疼痛程度均呈降低趋势,且观察组术后1个月、3个月、6个月VAS 评分(3。59±1。14)分、(1。94±0。63)分、(0。63±0。11)分均低于对照组(4。67±1。23)分、(2。57±0。85)分、(0。82±0。23)分(P<0。05);两组术后6个月髌骨倾斜角、髌骨移位距离、髌韧带比值、胫骨角均较术前减小(P<0。05),但组间比较无明显差异(P>0。05);术后6个月,两组髌骨功能、膝关节功能均得到改善,且观察组 Feller 髌骨评分、KSS 评分(20。37±1。29)分、(79。65±8。04)分较对照组(18。53±1。46)分、(74。12±9。31)分高,观察组OKS评分(19。30±1。14)分较对照组(21。67±1。85)分低,两组评分存在统计学差异(P<0。05)。结论 数字化3D打印导航模板辅助下的髌骨成形术治疗膝关节OA患者,能有效减轻膝前疼痛程度,改善髌骨及膝关节功能。
Application of digital 3D printing navigation template in patelloplasty
Objective To analyze the application effect of digital 3D printing navigation template in patelloplasty.Methods A total of 89 patients with knee osteoarthritis(OA)who needed total knee arthroplasty(TKA)in the First Affiliated Hospital of Henan University from June 2022 to June 2023 were selected and divided into two groups according to the random number table method.The control group(44 cases)was treated with traditional patelloplasty,and the ob-servation group(45 cases)was treated with patelloplasty assisted by digital 3D printing navigation template.The degree of anterior knee pain[visual analogue scale(VAS)]were compared between the two groups at different time points(be-fore operation,1 month,3 months and 6 months after operation),as well as imaging indexes(patellar tilt angle,patel-lar displacement distance,patellar ligament ratio,tibial angle),patellar function(Feller patellar score),knee joint function[American knee society score(KSS),oxford University knee score(OKS)].Results There was a statistically significant difference in the degree of anterior knee pain between the two groups at different time,between groups,time ×group(P<0.05);with the passage of time,the degree of anterior knee pain in the two groups showed a down-ward trend,and the VAS scores(3.59±1.14)points,(1.94±0.63)points,(0.63±0.11)points in the observation group were lower than those in the control group at 1 month,3 months,and 6 months after surgery(4.67±1.23)points,(2.57±0.85)points,(0.82±0.23)points(P<0.05);Six months after surgery,the patellar tilt angle,pa-tellar displacement distance,patellar ligament ratio,and tibial angle in both groups decreased compared to preoperative values(P<0.05),but there was no significant difference between the groups(P>0.05);Six months after surgery,both groups showed improvement in patellar function and knee joint function.The Feller patellar score and KSS score in the observation group(20.37±1.29)points,(79.65±8.04)points were higher than those in the control group(18.53±1.46)points,(74.12±9.31)points,while the OKS score in the observation group(19.30±1.14)points was lower than that in the control group(21.67±1.85)points.There was a statistically significant difference in scores between the two groups(P<0.05).Conclusion Patelloplasty assisted by digital 3D printing navigation template in the treatment of patients with knee OA can effectively reduce the degree of anterior knee pain,improve the function of patel-la and knee joint.
