基于关联规则和隐结构模型的白癜风古今用药规律研究

目的:挖掘分析古今中医药治疗白癜风的用药规律和配伍特点。方法:查阅古籍及检索中国知网(CNKI)、万方(Wanfang Data)、维普(VIP),筛选2013年1月1日至2023年1月1日期间有关白癜风治疗的文献,建立数据库,通过软件对中药进行频数分析、关联规则分析、隐结构模型分析、聚类分析及因子分析。结果:共筛选出方剂129首,中药286味,总使用频数1 316。古方以防风、附子、苦参等中药的使用频数较高;用药多属解表药、补虚药、祛风湿药等;药性多温、平、寒,药味多辛、苦、甘,主要入肝经、脾经、肺经;共挖掘高频中药的潜在药物组合19组;隐结构分析得到6个隐类,结合中医知识推论得到3个核心方剂;获得5个聚类方;共提取8个公因子。今方以当归、补骨脂、刺蒺藜等中药的使用频数较高;用药多属补虚药、活血化瘀药、解表药等;药性温、平、寒,药味甘、辛、苦,主要入肝经、脾经、肺经;共挖掘高频中药的潜在药物组合22组;隐结构分析得到10个隐类,结合中医知识推论得到5个核心方剂;获得5个聚类方;共提取9个公因子。结论:古代医家认为白癜风患病多为虫毒、风邪、湿邪,治疗以攻毒杀虫、祛风通络、健脾益肺、调气和血为主;当代医家认为白癜风患病多为脏腑功能失调,治疗以补肾益肺、健脾益胃、祛风通络、活血化瘀为主。
The Medication Rule of Ancient and Modern for Vitiligo Based on Association Rules and Latent Structure Model
Objective:To explore and analyze the medication rules and compatibility characteristics of ancient and modern prescriptions for clinical treatment of vitiligo.Methods:Clinical research literature about vitiligo was retrieved from ancient books and CNKI,Wanfang,and VIP from January 1,2013 to January 1,2023.And database was established.Frequency analysis,association rule analysis,latent structure model analysis,cluster analysis,and factor analysis on traditional Chinese medicine were carried out through software.Results:A total of 129 prescriptions and 286 traditional Chinese medicines were selected,with a total use frequency of 1316.In ancient prescriptions,the herbs with high usage frequency included Fangfeng(Saposhnikovia divaricate),Fuzi(Monkshood)and Kushen(Sophora flavescens)etc.And relieving exterior syndrome drugs,deficiency-nourishing d rugs and wind damp dispelling drugs were mainly used.In terms of nature,warm,plain and cold ranked among the top three.In terms of flavor,the proportion of spicy,bitter and sweet was the highest.In terms of meridian distribution,liver,spleen and lung were the most critical.A total of 19 groups of potential drug combinations of high-frequency Chinese medicine were excavated.Latent structure model analysis yields six latern variables,to infer three core prescriptions according to traditional Chinese medicine knowledge.A total of five clustering combinations can be formed based on clustering analysis of high frequency drugs.A total of 8 common factors were extracted.In modern prescriptions,the herbs with high usage frequency included Danggui(Angelica sinensis),Buguzhi(Fructus psoraleae)and Cijili(Tribulus terrestris)etc.And relieving deficiency-nourishing drugs,blood activating and stasis removing drugs and relieving exterior syndrome drugs were mainly used.In terms of nature,warm,plain and cold ranked among the top three.In terms of flavor,the proportion of sweet,spicy and bitter was the highest.In terms of meridian distribution,liver,spleen and lung were the most critical.A total of 22 groups of potential drug combinations of high-frequency Chinese medicine were excavated.Latent structure model analysis yields 10 latern variables,to infer five core prescriptions according to traditional Chinese medicine knowledge.A total of five clustering combinations can be formed based on clustering analysis of high frequency drugs.A total of nine common factors were extracted.Conclusion:The ancient doctors believed that vitiligo was mostly caused by insect poison,wind evil and damp evil.The main treatments are attacking toxin and killing insects,expelling wind and dredging collaterals,invigorating spleen and benefiting lung,and regulating Qi and blood.Contemporary doctors believe that vitiligo is mostly caused by visceral dysfunction,and the treatment is mainly based on tonifying kidney and lung,invigorating spleen and stomach,expelling wind and dredging collaterals,and promoting blood circulation and removing blood stasis.
