摘要
目的:观察壮医湿毒清洗剂治疗湿疮(湿热浸淫证)的临床疗效.方法:选取2021年4月至2022年9月就诊于广西中医药大学附属瑞康医院的湿疮(湿热浸淫证)患者80例,随机分为治疗组及对照组各40例.治疗组用壮药湿毒清洗剂湿敷,对照组用复方黄柏液湿敷,两组均以14 d为1个疗程.1个疗程后,比较两组患者的临床疗效,治疗前后中医证候积分、湿疹面积及严重度指数(EASI)、瘙痒程度评分、皮肤病生活质量指数(DLQI)评分,以及血清免疫球蛋白E(IgE)、肿瘤坏死因子-α(TNF-α)、白介素-4(IL-4)及IL-6表达水平.结果:治疗组和对照组的临床总有效率分别为83.33％(30/36)及54.29％(19/35),治疗组优于对照组(P＜0.05);治疗后,两组患者的中医证候积分、EASI评分、瘙痒程度评分、DLQI评分均较治疗前降低(P＜0.01),且治疗组患者中医证候积分、EASI评分及DLQI评分均低于对照组(P＜0.05);治疗后,两组患者血清中IgE、TNF-α、IL-4、IL-6水平均较治疗前降低(P＜0.05),且治疗组患者血清中IgE、TNF-α、IL-6水平均低于对照组(P＜0.05).结论:壮药湿毒清洗剂治疗湿疮(湿热浸淫证)有较好的临床疗效,且可明显降低患者血清中炎症因子IgE、TNF-α、IL-6、IL-4水平,这可能是壮药湿毒清洗剂治疗湿疮(湿热浸淫证)的机制之一.
Abstract
Objective:To observe the clinical efficacy of Zhuang medicine Shiduqing Lotion(湿 毒清洗剂)in the treatment of eczema(damp-heat immersion syndrome).Methods:Eighty patients with eczema(damp-heat immersion syndrome)who visited Ruikang Hospital Affiliated to Guangxi University of Chinese Medicine from April 2021 to September 2022 were selected and divided into two groups,40 cases in each according to the random number table method.The treatment group was treated with Zhuang medicine Shiduqing lotion wet compress,and the control group was treated with Compound Huangbai Liquid(复 方黄柏 液)wet compress.Both groups were treated for 14 days as a course of treatment.After one course of treatment,the total clinical efficiency,pre-and post-treatment TCM syndrome score,eczema area and severity index(EASI),pruritus score,dermatological quality of life index(DLQI)score,and serum IgE,TNF-α,IL-4 and IL-6 expression levels were compared between the two groups.Results:After treatment,the total clinical efficiency of the treatment group and the control group were 83.33％(30/36)and 54.29％(19/35),respectively,and the treatment group was better than the control group(P＜0.05).After treatment,the TCM syndrome scores,EASI scores,pruritus scores,and DLQI scores in two groups were decreased when compared with those before treatment(P＜0.01),and the TCM syndrome scores,EASI scores,and DLQI scores in the treatment group were lower than those in the control group(P＜0.05).After treatment,the serum levels of IgE,TNF-α,IL-4,and IL-6 in two groups were significantly decreased(P＜0.05),and the serum levels of IgE,TNF-α,and IL-6 in the treatment group were significantly lower than those in the control group(P＜0.05).Conclusion:Zhuang medicine Shiduqing lotion has a good clinical effect in the treatment of eczema(damp-heat immersion syndrome),and can significantly reduce the levels of inflammatory factors IgE,TNF-α,IL-6 and IL-4 in the serum of patients,which may be one of the mechanisms of Zhuang medicine Shiduqing lotion in the treatment of eczema(damp-heat immersion syndrome).
