摘要

射血分数保留型心力衰竭是休息或运动时左室充盈压增高,出现以心力衰竭症状或体征为表现的一种临床综合征.心外膜脂肪组织(EAT)对射血分数保留型心力衰竭的发生发展起着关键作用,目前尚无特效药物治疗.从"结"病的视角认识射血分数保留型心力衰竭合并心外脂肪组织,发现微血管炎症与心脏底物利用和能量代谢是其核心病机.基于"结者散之"理论,探讨通过散结减轻炎症反应、维持心肌能量稳态、抑制心肌纤维化,进而对射血分数保留型心力衰竭发挥积极治疗作用.

Abstract

Heart failure with preserved ejection fraction(HFpEF)is a clinical syndrome characterized by increased left ventricular filling pressure at rest or exercise and the appearance of symptoms or signs of heart failure.It has been well established that epicardial adipose tissue(EAT)plays a critical role in the development of HFpEF,and there is no specific treatment for this condition.From the perspective of"accumulation"disease,it was found that microvascular inflammation,substrate utilization,and energy metabolism were the core patho-genesis of HFpEF combined with EAT.Based on the"treating pathogenic accumulation with dissipation"theory,this article explored the positive therapeutic effect of HFpEF by dispersing knot to reducing inflammatory response,maintaining myocardial energy homeostasis,and inhibiting myocardial fibrosis.
