Effect of Shenling Baizhu San(参苓白术散)on Intestinal Inflammation and PI3K/Akt Signal Pathway in Crohn's Disease Rats
Objective:To investigate the mechanism of Shenling Baizhu San in alleviating colonic inflammation in rats with Crohn's disease(CD)and its effect on phosphatidylinositol 3-kinase/protein kinase B(PI3K/Akt)signaling pathway.Methods:A total of 54 SD rats were randomly divided into blank group,model group,mesalazine group(0.21 g/kg),Shenling Baizhu San low-dose group(5.88 g/kg),Shenling Baizhu San medium-dose group(11.76 g/kg),and Shenling Baizhu San high-dose group(23.59 g/kg),with 9 rats in each group.5%2,4,6-trinitrobenzenesulfonic acid(TNBS)enema was used for CD modeling.The drug intervention group was given corresponding treatment factors after successful modeling,and the blank group and model group were given the same dose of normal saline.After 14 days,the pathological changes of the colon surface of the rats were observed in each group,and the colonic mucosal injury index(CMDI)score was obtained.Hematoxylin-eosin(HE)staining was used to observe the histopathological changes of the colon of rats.Western blotting was used to detect the protein expression levels of phosphatidylinositol 3-kinase(PI3K),phosphoryl PI3K(p-PI3K),protein kinase B(Akt)and phosphorylated Akt(p-Akt)in the sampled intestinal mucosal tissues.Results:The model group showed higher CMDI score than blank group(P<0.01).The mesalazine group,Shenling Baizhu San medium-dose group and Shenling Baizhu San high-dose group showed lower CMDI score than model group(P<0.01).The mucosal tissue damage was not obvious in blank group,and the structure and tissue were intact.There was no obvious hyperemia and edema in blank group.Compared with the blank group,a large number of mucosal epithelial cells were necrotic and shed in model group,and goblet cells were signifi-cantly reduced.Single-tubular glands were destroyed and the sequencing was disordered in model group and a large number of inflammatory cells and immune lymphocyte infiltration appeared in each layer in model group.Lymphotic tissue proliferation and lymphatic follicles appeared in model group.The mesalazine group,Shenling Baizhu San low-dose group,Shenling Baizhu San medium-dose group and Shenling Baizhusan high-dose group showed better recovery degree of mucosal injury,than model group to varying degrees,and the mucosal epithelial cells of colon tissue were not significantly shed and necrotic.The infiltration of inflammatory cells was less,and the glandular destruction was slight in mesalazine group,Shenling Baizhu San low-dose group,Shenling Baizhu San medium-dose group and Shenling Baizhusan high-dose group.The normal structure was not completely lost.There was a small amount of vasodilation and bleeding and a slight inflammatory reaction in Shenling Baizhu San low-dose group.The model group showed higher p-PI3K/PI3K and p-Akt/Akt in colon tissues than blank group(P<0.01).Mesalazine group,Shenling Baizhu San low-dose group,Shenling Baizhu San medium-dose group and Shenling Baizhusan high-dose group showed lower p-PI3K/PI3K and p-Akt/Akt in colon tissues than model group(P<0.05 or P<0.01).Conclusion:Shenling Baizhu San can relieve intestinal inflammatory reaction in CD rats,and its mechanism may be related to the inhibition of PI3K/Akt signal pathway.
