基于辨机论治分析类风湿关节炎相关肺间质病变，认为该病属于"肺痹"范畴，可分为三期。肺痹早期病机为外邪侵袭;中期病机为肺气虚损，肝木不疏，痰瘀互结;晚期病机为肺肾亏虚，毒损肺络;同时药毒病机贯穿疾病始终。治疗方面，早期当因势利导，宣肺散邪;中期当补益肺气，疏理肝气，化痰逐瘀;晚期当补益肺肾，解毒通络，同时应尽量控制药毒损害。
Analysis of Diagnosis and Treatment of Rheumatoid Arthritis-Interstitial Lung Disease Based on Mechanism Differentiation and Treatment
This article analyzed the rheumatoid arthritis-interstitial lung disease(RA-ILD)based on the mechanism differentiation and treatment.It is considered that the disease belongs to the category of"Fei Bi"and can be divided into three stages.The early pathogenesis of Fei Bi was the invasion of external pathogens,the middle pathogenesis was lung Qi deficiency,unsmooth of liver Qi and phlegm and blood stasis,and the late pathogenesis was lung and kidney deficiency and toxic damage to lung collaterals.At the same time,drug toxicity pathogenesis runs through the disease.In terms of treatment,early treatment was to dispele pathogens as soon as possible,disperse lung and eliminate pathogens,and middle treatment was to replenish and restore pulmonary Qi,regulate and soothe liver Qi,resolve phlegm and remove blood stasis,and late treatment was to replenish lung and kidney,detoxify and dredge collaterals,and pharmaco toxic damage should also be controlled as much as possible.
