摘要

目的:通过数据挖掘技术和网络药理学分析国医大师李佃贵教授治疗复发性阿弗他溃疡(RAU)的用药规律,筛选核心处方并对其潜在机制进行分析,以期为临床治疗RAU提供思路及理论依据.方法:收集2021年1月至2023年7月李佃贵教授治疗RAU患者的门诊处方,运用SPSS Statistics 26.0、SPSS Modeler 18.0分析软件对筛选出的中药处方用药规律进行研究,并运用Cytoscape 3.9.1进行复杂网络的可视化,确定核心处方并通过网络药理学分析其治疗RAU的成分、靶点、作用通路等.结果:共纳入医案156则,录入有效中药处方103份,共计164味中药,总频数为2 121.药物频数较高的是黄连、生地黄、黄芩、柴胡、清半夏、川牛膝、淡竹叶、蒲公英等,药性以寒、温居多,药味以苦、甘、辛为主,归经主要为肺、肝、胃经.核心处方为黄连、生地黄、黄芩、柴胡、清半夏,其重要活性成分有槲皮素、豆甾醇、山柰酚和生物素酰基-4-氨基丁酸等,关键靶点包括肿瘤坏死因子(TNF)、白细胞介素-6(IL-6)、白细胞介素-1β(IL-1β)、γ干扰素(IFN-γ)等,关键通路有PI3K-Akt信号通路、ACE-RACE信号通路等.结论:李佃贵教授认为浊毒内蕴是RAU的核心病机,并以清热化湿、泄浊解毒法治疗RAU.核心处方可能通过PI3K-Akt信号通路、ACE-RACE信号通路起到治疗RAU的作用.

Abstract

Objective:By using data mining techniques and network pharmacology research methods,this study analyzes the medication patterns of Professor LI Diangui,a master of traditional Chinese medicine,in the treatment of recurrent aphthous ulcer(RAU),screens core prescriptions,and analyzes their potential mechanisms,in order to provide ideas and theoretical basis for clinical treatment of RAU.Methods:The outpatient prescriptions of Professor LI Diangui for treating RAU patients from January 2021 to July 2023 were collected.And SPSS Statistics 26.0 and SPSS Modeler 18.0 analysis software was used to study the medication patterns of the selected traditional Chinese medicine prescriptions.Cytoscape 3.9.1 was used to visualize the complex network,then to determine the core prescription,and analyze its components,targets,and pathways of action for treating RAU through network pharmacology.Results:A total of 156 medical cases were included,and 103 effective traditional Chinese medicine prescriptions were recorded,with a total of 164 traditional Chinese medicines and a total frequency of 2 121 times.The drugs with higher frequency include Huanglian(Coptidis Rhizoma),Shengdihuang(Rehmanniae Radix),Huangqin(Scutellariae Radix),Chaihu(Bupleuri Radix),Qingbanxia(Pinelliae Rhizoma Praeparatum Cum Alumine),Chuanniuxi(Cyathulae Radix),Danzhuye(Lophatheri Herba)and Pugongying(Taraxaci Herba),etc.The medicinal properties are mostly cold and warm,and the medicinal flavors are mainly bitter,sweet,and spicy.The meridians mainly belong to the lung,liver,and stomach.The core prescriptions are determined,including Huanglian(Coptidis Rhizoma),Shengdihuang(Rehmanniae Radix),Huangqin(Scutellariae Radix),Chaihu(Bupleuri Radix)and Qingbanxia(Pinelliae Rhizoma Praeparatum Cum Alumine).The important active ingredients include quercetin,stigmasterol,kaempferol,and biotinyl-4-aminobutyric acid.The key targets include TNF,IL-6,IL-1β,and IFN-γ,key pathways include PI3K-Akt signaling pathway,ACE-RACE signaling pathway,etc.Conclusion:Professor LI Diangui believes that the accumulation of turbid toxins is the core pathogenesis of RAU,and the treatment of RAU is carried out through the methods of clearing heat,dispelling dampness,and detoxifying turbidity.The core prescription may play a role in treating RAU through the PI3K-Akt signaling pathway and ACE-RACE signaling pathway.
