An Analysis of Manipulation and Point Selection Rules of Massage Therapy for Lumbar Spinal Stenosis
Objective:To analyze the manipulation and point selection rules of massage therapy for lumbar spinal stenosis.Methods:The clinical research literature on the treatment of lumbar spinal stenosis with tuina in China Knowledge Information Network(CNKI),Wanfang Data Knowledge Service Platform(Wanfang Data),China Biomedical Literature Database(CBM),and PubMed until September 29,2023 were searched.The database of tuina prescription was established after checking and screening.R4.2.3 software was used to perform descriptive analysis,association rule analysis,and cluster analysis on the data.Results:A total of 106 clinical research papers on the treatment of lumbar spinal stenosis with tuina were included,involving 46 tuina techniques with a frequency of 522 applications,of which the technique with the highest frequency of application was the Gun method.A total of 78 prescriptions foracupoints were extracted,involving 65 acupoints with a frequency of 614,and the acupoint with the highest frequency of application was Huantiao(GB30).The acupoints were mainly concentrated in the bladder meridian,gallbladder meridian,governor vessel,stomach meridian,and extraordinary points.In terms of the compatibility of techniques and acupoints,the most common methods are the combination of point pressing and Huantiao(GB30)with other techniques and acupoints.The network analysis of association rules showed that there were strong associations between Huantiao(GB30),Weizhong(BL40),Yanglingquan(GB34),and Chengshan(BL57),and strong associations between point-pressing,diagonal wrenching,flicking,rubbing,and pinching.Cluster analysis showed that each of the acupoints and maneuvers formed five effective clusters.Conclusion:Tuina therapy for lumbar spinal stenosis is mainly based on tendon loosening and bone-setting techniques,emphasizing the principle of"equal emphasis on tendons and bones",focusing on local and meridian-based acupoints,and giving full play to the synergistic effect of the manipulation tech-niques and acupoints.
