摘要
目的:总结分析针灸治疗肾性水肿的选穴规律.方法:检索中国知网、万方数据库、维普数据库、中国生物医学文献数据库、PubMed数据库中针灸治疗肾性水肿的文献,提取数据后采用Microsoft Excel 2019建立腧穴处方数据库,采用SPSS Modeler 18.0和SPSS 26.0软件进行数据分析.结果:最终纳入99篇文献,涉及腧穴74个,应用总频数为546.针灸治疗肾性水肿应用频数排名前5位的腧穴分别为肾俞、足三里、三阴交、关元、水分;腧穴主要归属的经脉为任脉、足太阳膀胱经、足太阴脾经和足阳明胃经;腧穴分布以下肢部腧穴最多,共涉及25个;特定穴中五输穴使用频数最高.腧穴强关联规则17条,其中"脾俞→肾俞"支持度最高;聚类分析结果显示腧穴分为4类;核心腧穴为脾俞、肾俞、阴陵泉、三阴交、足三里、气海、关元、水分.结论:针灸治疗肾性水肿选穴以任脉、足太阳膀胱经、足太阴脾经和足阳明胃经腧穴为主,多以平调阴阳、培土制水为指导思想进行辨证论治,并注重温化水饮和调畅三焦.
Abstract
Objective:To summarize and analyze the rule of acupoint selection of acupuncture and moxibustion in the treatment of renal edema.Methods:The literature of acupuncture and moxibustion in the treatment of renal edema in CNKI database,Wanfang database,VIP database,Chinese biomedical literature database and PubMed database were searched.After extracting the data,Microsoft Excel 2019 was used to establish the acupoint prescription database,and SPSS Modeler 18.0 and SPSS 26.0 software were used for data analysis.Results:99 articles were ultimately included,involving 74 acupoints,with a total application frequency of 546.The top 5 acupoints in the application frequency of acupuncture and moxibustion in the treatment of renal edema were Shenshu(BL 23),Zusanli(ST 36),Sanyinjiao(SP 6),Guanyuan(RN 4),and Shuifen(RN 9).The main meridians that acupoints belong to were the Ren meridian,the Foot Taiyang Bladder meridian,the Foot Taiyin Spleen meridian,and the Foot Yangming Stomach meridian.The distribution of acupoints in the lower limbs is the highest,involving a total of 25 acupoints.Among specific acupoints,the five-shu acupoints have the highest frequency of use.There were 17 strong association rules for acupoints,among which"Pishu(BL 20)→ Shenshu(BL 23)"has the highest support.The results of cluster analysis showed that the acupoints were divided into 4 categories.The core acupoints were Pishu(BL 20),Shenshu(BL 23),Yinlingquan(SP 9),Sanyinjiao(SP 6),Zusanli(ST 36),Qihai(RN 6),Guanyuan(RN 4),Shuifen(RN 9).Conclusion:The acupoints of Ren Meridian,Foot Taiyang Bladder Meridian,Foot Taiyin Spleen Meridian and Foot Yangming Stomach Meridian are the main acupoints selected for acupuncture and moxibustion treatment of renal edema,and the guiding ideology of syndrome differentiation and treatment is to regulate Yin and Yang,cultivating the earth to treat edema,and pay attention to warming water and regulating the triple energizer.
