Staging Treatment of Idiopathic Pulmonary Fibrosis Based on"Membrane-Interstice-Three jiao"Theory
The theory of"membrane-interstice-three jiao"was put forward by Professor CHEN Chaozu of Chengdu University of TCM.He believed that the three jiao consists of the membrane and the interstice,and it is a channel for the operation of qi,blood,essence,and body fluid,which affects the metabolism and energy flow of the whole body.The pathological evolution of idiopathic pulmonary fibrosis(IPF)and"lung paralysis-lung wither"disease process transition characteristics are highly compatible.We believe that the membrane-interstice disharmony and the three jiao imbalance are throughout the IPF.During the inflammatory period,chronic diseases can develop into lung deficiency,making the patient more susceptible to other pathogenic qi,resulting in membrane-interstice discomfort.During fibrosis formation,phlegm,blood stasis,and toxin block the membrane-interstice,causing its stiffness.During the fibrosis period,the membrane-interstice is damaged,and the three jiao withers until it fails to function normally.This article emphasizes regulating the membrane-interstice,dredging the three jiao of the basic principles,phased use of"dredge,eliminating,tonifying"three primary treatments:firstly,to eliminate the evil qi in lungs,and dredge the membrane-interstice;secondly,to eliminate the product of excess,and reduce the spasms;thirdly,to promote wound recovery,and nourish the body.The combination of TCM pathogenesis and modern pathology enriches the theoretical connotation of TCM in treating IPF.
